慢性肝炎重度和慢性重型肝炎患者红细胞及血小板参数变化及意义

目的　了解红细胞及血小板参数变化在慢性肝炎患者病情变化中的意义。方法　采用负压采血管分别采集患者及健康对照者外周血 ,以全自动血球分析仪检测红细胞及血小板参数。结果　两组病例组RBC、Hb、PLC、PCT值均显著低于健康对照组 (p <0 .0 1) ,而MCV、PLC、PCT值均显著低于慢性重型肝炎组 ,MCV、RDW、MPV值均显著高于慢性重型肝炎组 (p <0 .0 1) ;慢性重型肝炎早期PLC、PCT值均显著低于慢性重型肝炎中期 ,PDW值均显著高于对照组 (P <0 .0 1) ,并与肝脏损害程度呈正相关。应用小剂量糖皮质激素后随TBIL水平的下降Hb、PLC、PCT逐渐升高 ,而RDW、MPV、PDW逐渐下降 ,用药前与用药后相比 ,P <0 .0 1。结论　红细胞及血小板部分参数可作为判断慢性肝炎病情的参考指标

Some red blood cell and platelet parameter changes in patients with chronic hepatitis B of severe degree and with chronic severe hepatitis B

Objective To investigate the significance of some red blood cell and platelet parameter changes in patients with chronic hepatitis B of severe degree and with chronic severe hepatitis B.Methods Some red blood cell and platelet parameters were counted by CD 1700 Diagnostics in patients with chronic hepatitis B of severe degree ,with chronic severe hepatitis B and in healthy individuals for control.Results The RBC,Hb,PLC and PCT were lower in patients with chronic hepatitis B of severe degree and with chronic severe hepatitis B than in control ( p <0.01),while MCV,RDW,MPV and PDW higher in the two patient groups than in control( p <0.01);RBC,Hb,PLC and PCT in patients with chronic hepatitis B of severe degree were lower than in patients with chronic severe hepatitis B,while MCV,RDW and MPV higher than in chronic severe hepatitis B patients( p<0.01 );PLC and PCT in early stage of chronic severe hepatitis B were lower than in middle stage of these patients;The Hb,PLC and PCT increased,while RDW,MPV and PDW decreased as total serum bilirubin levels decreased after low dose of steroid administration.Conclusion Some red blood cell platelet parameters are valuable in predicting hepatitis B patients with hyperbilirubinemia.
