幽门弯曲菌与胃炎,溃疡病的关系：附134例研究

对134例临床患者,胃镜直视下采粘膜标本做以下检测:1.应用CP试剂做尿素酶试验;2.按常规切片,HE染色,病理检查后再用1000倍油镜检菌。检测发现:1.消化道病CP阳性率为53.6％,显著高于正常粘膜11.11％(P<0.01);2.胃炎与溃疡急性活动期,CP阳性率69.44％,显著高于非活动期28.57(P<0.01);3.胃溃疡、十二指肠溃疡及复合性溃疡,CP阳性率分别为75.5％、54.0％、100％。其中2例多发性溃疡及3例复合性溃疡,CP阳性率为100％。4.对CP阳性的胃炎及溃疡患者,随机抽样25例,选用CP敏感庆大霉素治疗4周,其症状缓解率为84.0％,活动性炎症转为非活动性68.0％,CP转阴率88.0％,均显著高于对照组。结果提示:1.CP感染与上消化道慢性炎症及溃疡有相关性。2.加CP敏感的抗生素治疗有显著疗效。3.CP感染可能是上消化道慢性炎症与溃疡病的原因之一。

RELATIONSHIP BETWEEN GASTRITIS PEPTIC ULCER AND CAMPYLOBACTER PYLORIDIS

Relationship between gastritis, peptic ulcer and cam- pylobacter pyloridis (CP) was studied in 134 cases.Mucosal specimens were obtained under endoscope and tested with CP reagent.Bacteria were searched for under 1,000 power oil microscope after the specimens were treated by routine section, HE staining and pathological examin- ation. The results are 1) CP positive rate of gas tric mucosa in upper gastrointestinal diseases was 53.6%, prominently higher than that of nornal mucosa (11.11%, P<0.01) ; 2) CP positive rate in acute period of gastritis and peptic ulcer was 69.44%, prominently higher than that in inactive period (28.57%, P<0.01) ; 3) CP positive rates of gastric ulcer duodenal ulcer and compound ulcer were 75%, 54.0%and 100% res- pectively, in which two compound ulcer and three multiple ulcer were 100%; 4) 25 random sampling cases in CP positive cases of gastritis and peptic ulcer were treated with gentamycin for 4 weeks, to which CP were sensitive. The active inflammations turn to inactive ones by 68.0% and CP turnsto negative ones by 88.0%,obviously higher than control group. The results showed that: 1)there is relation between CP infec- tion and upper gastrointestinal inflammation or ulcer; 2) Antibiotics are effective. 3) CP infection may be one of the causes ofupper gastro- chronic inflammation and ulcer of upper digestive tract.
