综合治疗退行性下颈椎不稳症并颈性眩晕的临床探讨

目的 通过临床治疗退行性下颈椎不稳症并颈性眩晕的疗效,探讨颈椎失稳与颈性眩晕的发病关系.方法 回顾性研究退行性下颈椎不稳症并颈性眩晕64例,随机分为两组,A组采用静滴生脉针和红花针,口服倍他司汀口服液；B组在A组基础上行床边颈椎牵引,两组疗效欠满意者采用手术治疗.检测治疗前后椎动脉血流变化及对疗效进行评价,统计分析.结果 治疗后椎动脉血流( Vs、Vd、RI)比治疗前有所改善,但差异无统计学意义(P＞0.05)；A组有效率为56.25％,B组有效率为81.25％,B组疗效优于A组(P＜0.05),差异有统计学意义.结论 颈椎失稳是颈性眩晕发病的重要原因之一,维持颈椎的稳定性是治疗退行性下颈椎不稳症并颈性眩晕的重要方法.

Comprehensive treatment for degenerative instability of lower cervical spine and cervical vertigo

Objective To explore the relationship between cervical spine instability and cervical vertigo by analyzing the efficacy of therapies for degenerative instability of lower cervical spine and cervical vertigo. Methods The data on 64 with degenerative instability of lower cervical spine and cervical vertigo were retrospoctively analyzed. All the patients were randomly assigned to receive intravenous Skengmai injection, Honghua injection, and oral betahistine Hydrochloride solution （ group A ）or bedside cervical traction in addition to the therapies in the group A （ group B ）. The patients without ideal efficacy received surgical treatment. The changes in vertebral artery blood flow were detected and the efficacy was assessed. Results As compared with the baseline, vertebral artery blood flow did not change significantly after treatment （ P〉 0.05 ）. Group B was superior to group A in the effectiveness rate （ 81.25% vs. 56.25%, P 〈 0.05 ）. Conclusions Cervical spine instability is one of the important eauses of cervical vertigo. Maintenance of cervical spine stability is an important method tor the treatment of degenerative instability of lower cervical spine and cervical vertigo.
