摘    要:目的探讨以蓝染法行乳腺癌前哨淋巴结活检(SLNB)的可行性。方法收集巢湖市第一人民医院2009年3月~2012年10月行SLNB的乳腺癌患者86例,均以单纯蓝染法行SLNB,采用检出率和假阴性率对活检情况进行评价,分析影响SLNB检出率及假阴性率的相关因素。结果86例患者中79例成功检出前哨淋巴结(SLN),检出率为91.86%,假阴性率为9.70%(3/31)。新辅助化疗、体重指数(BMI)、注射染料前皮下注射肾上腺素与SLN的检出率有关,差异有统计学意义(P〈0.05);BMI、检出SLN的个数与SLN假阴性率有关,差异有统计学意义(P〈0.05)。结论以单纯蓝染法行前哨淋巴结活检的检出率和假阴性率完全可以达到(2011中国抗癌协会乳腺癌诊治指南与规范》的要求.是安全、可行的。

Abstract:Objective To explore the feasibility of blue dye method used in the sentinel lymph node biopsy of breast cancer. Methods 86 patients of breast cancer in the First People's Hospital of Chaohu City from March 2008 to Octo ber 2012 were collected. All patients were performed sentinel lymph node biopsy (SLNB) with simple blue dye method. Detection ratio and false negative rate were used to evaluate the biopsy cases, the relevant factors on the de tection ratio and false negative rate of SLNB were analyzed. Results Sentinel lymph node (SLN) were checked out in 79 cases of 86 patients, detection ratio was 91.86%, false negative rate was 9.70% (3/31). Neoadjuvant chemotherapy, body mass index (BM1), epinephrine subcutaneous injected before the dye injection were found had correlation with SLN detection ratio, the differences were statistically significant (P 〈 0.05); BMI, the number of SLN were found had correlation with false negative rate of SLNB, the differences were statistically significant (P 〈 0.05). Conclusion The detection rate and false negative rate of simple blue dye method used in SLNB can reach the guidelines and norms of breast cancer's diagnosis and treatment which released by 2011 Chinese anti-cancer association. It is safe and feasible.
