高屈膝体位在股骨及髌骨骨折术后的应用研究

目的探讨股骨及髌骨骨折患者术后早期采用高屈膝体位的可行性及临床意义。方法选用2010年1月～2012年10月的49例股骨及髌骨骨折患者，在术后5～7d内将患肢膝关节间断放置于≥105°的高屈膝体位．并予以止痛治疗及个性化的舒适护理，5～7d后嘱患者加强患侧膝关节的关节活动度锻炼。结果本组49例患者在术后5～7d内均能按高屈膝体位放置的要求进行放置，术后伤口全部愈合良好，从膝关节的关节活动范围看，术后除l例膝关节有伸直受限外，48例患者膝关节伸直均达到00，术后1个月膝关节屈曲角度≥145°甚至正常活动的患者有43例，膝关节平均屈曲角度为143°。结论股骨及髌骨骨折患者在术后5～7d内间断应用高屈膝体位放置的治疗方法，结合有效的镇痛方案，有利于膝关节功能的早日恢复，值得推广应用。

Study of high knees posture application after femur and patella fracture surgery

Objective To discuss the feasibility of early application of high knees posture after femur and patella fracture surgery and its clinical significance. Methods 49 cases of patients with femoral and patella fracture from January 2010 to October 2012 were selected. Patients＇ knee-joints of affected limbs were put ≥145° high knees posture discontinuously 5-7 d after operation, and analgesia treatment and personalized comfortable nursing were given. 5-7 d after operation, patients were be asked to strengthen knee joint activity exercise of affected limbs. Results All 49 patients in this study obeyed high knees posture placed requirements 5-7 d after operation, the wound healing were all good, the range of motion of knee joint showed that after operation except 1 case of knee joint had stretched limited, knee joint extension of 48 cases all reached 0°, 1 month after operation, knee buckling angles of 43 cases were reached ≥145° even normal activities, the average buckling angle of knee joints was 143~. Conclusion Treatment methods of discontinuously application of high knees posture 5-7 d after femur and patella fracture surgery combined with effective analgesic scheme, are conducive to the early recovery of knee joint function, which is worthy of popularization and application.
