中段尿病原菌的分离及耐药性

摘    要:目的分析泌尿系感染的病原菌分布及耐药性特点,为临床选用抗菌药物提供依据。方法按照《全国临床检验操作规程》要求操作,采用黑马DL-96半自动细菌鉴定仪鉴定菌种及药敏,对临床分离的465株泌尿系感染病原菌进行药物敏感试验。结果在分离的465株病原菌中,革兰阴性菌占77.8%、革兰阳性菌占14.7%;真菌占7.5%,革兰阴性菌以大肠埃希菌为主,革兰阳性菌以肠球菌属为主;检出产超广谱β内酰胺酶(ESBLs)菌128株,其中大肠埃希菌产ESBLs120株,占40.5%;肺炎克雷伯菌产ESBLs8株,占22.8%;MRS占12株,占葡萄球菌属42.8%,11株HLAR占肠球菌的36.7%。结论泌尿系感染的病原菌分布广泛,大部分菌株具有多重耐药性;建议临床医生重视细菌学培养,依据药敏试验结果指导临床合理用药。

Abstract:Objective To analyze the pathogenic bacteria distribution and their drug resistance in urinary system infection, and to provide the information for choosing suitable antibiotic. Methods totally 496 strains of pathogenic bacteria derived from the clinical urinary system infection were analyzed according to the require ments described in the National Regulations on Clinical Laboratory Operations. The bacteria were identified by the automated analyze HEIMA DL--96. Results In the 465 strains of bacteria 77.8% were Gram-negtive , 14.7% were Gram-positive and 7.5% were yest. Most of Gram-negtive bacteria were E.col and most of Gram-negtive bacteria were 128 strains produced extended spectrum beta- laetamases(ESBLs). 40.5%(120 bacteria) of them were E.eol and 22.8% percent(8 bacteria) were K.pen. 12 strains were MRS and they accounted for 42.8%, 11 strains in Enteroeoccus resisted to high concentration gentamicin and they accounted for 36.7% of Entcrococcus. Conclusions The pathogenic bacteria in urinary system infection distribute extensively and so me of them are multiple drug resistance, so we suggest that the clinicians pay more attentions to the bacteriology culture and choose the suitable antibiotic according to the Results of antibiotic susceptibility tests~
