成都地区45~60岁健康体检人群肿瘤标志物筛查结果分析

摘    要:目的分析成都地区45~60岁健康体检人群肿瘤标志物糖类抗原724(CA-724)、糖类抗原153(CA-153)、糖类抗原125(CA-125)、糖类抗原199(CA-199)、癌胚抗原(CEA)筛查结果。方法选取成都地区2019年5月至2020年8月45~60岁健康体检者2268例,采用电化学发光法检测其血清肿瘤标志物CA-724、CA-153、CA-125、CA-199、CEA水平,统计各项肿瘤标志物检测阳性率、恶性肿瘤确诊情况,将肿瘤标志物检测阳性者记为阳性组,阴性者记为阴性组,比较其各项血清肿瘤标志物水平,并分析不同性别、年龄的体检人群肿瘤标志物筛查结果,分析各项肿瘤标志物检测对恶性肿瘤的诊断效能。结果2268例健康体检者中,CA-724、CA-153、CA-125、CA-199、CEA阳性检出率分别为10.05%、4.98%、1.10%、7.01%、4.76%,总共72例肿瘤标志物检测阳性,总阳性率为3.17%(72/2268),其中12例经病理学、影像学等检查确诊为早期或中期肿瘤,肿瘤总检出率为0.53%(12/2268),其中恶性肿瘤8例,检出率为0.35%(8/2268),肿瘤标志物阳性组的CA-724、CA-153、CA-125、CA-199、CEA水平高于阴性组(P<0.05);男性CA-125、CA-199阳性率低于女性,而CEA阳性率高于女性(P<0.05);不同年龄人群CA-153、CA-199、CEA阳性率比较差异有统计学意义(P<0.05),成都地区健康体检人群中CA-153及CA-199阳性表达集中在51~60岁,CEA阳性表达集中在45~50岁;ROC曲线分析显示,CA-724、CA-153、CA-125、CA-199、CEA联合检测预测恶性肿瘤的灵敏度、特异度、准确度最高,曲线下面积最大,为0.865。结论成都地区45~60岁健康体检人群肿瘤标志物CA-724、CA-153、CA-125、CA-199、CEA均呈高表达,其中CA-724阳性率最高,不同性别、年龄人群的肿瘤标志物水平也有差异,联合检测各项肿瘤标志物有利于早期发现疾病,早期治疗,减少疾病恶化风险。

Abstract:Objective To analyze the screening results of tumor markers carbohydrate antigen 724(CA-724),carbohydrate antigen 153(CA-153),carbohydrate antigen 125(CA-125),carbohydrate antigen 199(CA-199)and carcinoembryonic antigen(CEA)in the 45~60-year-old health examination population in Chengdu area.Methods A total of 2268 healthy people aged 45~60 years old living in Chengdu area from May 2019 to August 2020 were selected.Levels of serum tumor markers CA-724,CA-153,CA-125,CA-199 and CEA were detected by electrochemiluminescence.Positive rates of different tumor markers and confirmed diagnosis of malignant tumors were statistically analyzed.People with positive and negative tumor markers were respectively included in the positive group and the negative group,and their serum tumor markers were compared.The screening results of tumor markers in the health examination population of different gender and age,and the diagnostic efficiency of each tumor marker for malignant tumors were analyzed.Results Among the 2268 healthy people,the positive rates of CA-724,CA-153,CA-125,CA-199 and CEA were 10.05%,4.98%,1.10%,7.01%and 4.76%,respectively.A total of 72 cases were positive for tumor marker detection,with a total positive rate of 3.17%(72/2268).Among them,12 cases were diagnosed as early or mid-term tumors by pathology,imaging and other examinations,with a total tumor detection rate of 0.53%(12/2268),including 8 cases of malignant tumors,with a detection rate of 0.35%(8/2268).The levels of CA-724,CA-153,CA-125,CA-199 and CEA in the tumor markers positive group were higher than those in the negative group(P<0.05).The CA-125 and CA-199 positive rates in males were lower than those in females,while CEA positive rate was higher in males than in females(P<0.05).Positive rates of CA-153,CA-199 and CEA in different age groups showed statistically significant differences(P<0.05).Positive expression of CA-153 and CA-199 mainly occurred in the 51~60 years old group,and positive expression of CEA mainly occurred in the 45~50 years old group.ROC curve analysis showed that combined detection of CA-724,CA-153,CA-125,CA-199 and CEA had the highest sensitivity,specificity and accuracy in predicting malignant tumors,and the area under the curve was the largest(0.865).Conclusion Tumor markers CA-724,CA-153,CA-125,CA-199 and CEA are highly expressed in the 45~60-year-old health examination population in Chengdu area,and positive rate of CA-724 is the highest.Levels of tumor markers in people of different gender and age are also different.Joint detection of various tumor markers is conducive to early detection and treatment of the disease,and reduction of deterioration risk.
