单侧经皮椎体成形术治疗老年骨质疏松性椎体压缩骨折

目的观察单侧经皮椎体成形术(PVP)治疗老年骨质疏松性椎体压缩骨折的临床疗效。方法选取57例(71个椎体)骨质疏松性椎体压缩骨折患者进行单侧PVP术治疗。观察术前、术后72 h、6个月疼痛VAS评分及椎体前缘、中间高度、椎体后凸Cobb角变化。结果患者均获随访6个月,除3例疼痛恢复不明显外,其余患者术后疼痛均缓解。VAS评分:术前(8.10±0.82)分,术后72 h(3.12±1.13)分,术后6个月(2.83±0.90)分,术后72 h、6个月与术前比较差异有统计学意义(P0.05),术后两时段比较差异无统计学意义(P0.05)。与术前比较,手术后X线片测量椎体前缘、中间高度、椎体后凸Cobb角有改善,但差异无统计学意义(P0.05)。结论 PVP术操作简单、创伤小、效果明显,是治疗老年骨质疏松性椎体压缩性骨折安全、有效的方法。

Treatment of vertebral compressive fractures in aged patients with osteoporosis by unilateral percutaneous vertebroplasty

Objective To observe the effect of unilateral percutaneous vertebroplasty （ PVP） in the treatment of osteo-porotic vertebral compressive fractures in aged patients. Methods Unilateral pedicle PVP were performed in 57 cases （71 vertebrae） of osteoporotic vertebral compressive fractures. Visual analogue scores（VAS）,the heights of anterior border and middle region of the vertebral body,and vertebral kyphosis angle （ Cobb angle） before operation,72 hours and 6 months after operation were observed. Results Patients were followed up for 6 months,low back pain relieved obviously in 54 cases postoperatively,but there was little effect in other 3 cases. VAS was reduced from a mean preop-erative score of 8. 10 ± 0. 82 to a mean postoperative score of 3. 12 ± 1. 13 at 72 hours after operation or 2. 83 ± 0. 90 in 6 months after operation respectively. Compared with before operation, VAS showed significant difference in 72 hours,6 months after operation（P〈0. 05）,but not among those two segments after operation（P〉0. 05）. The heights of anterior border,middle region of the vertebral body and Cobb angle indicated by X ray had improved a little after operation,but without statistical significance（P〉0. 05）. Conclusions Unilateral PVP is a simple and minimally in-vasive surgery,it is a safe and effective method for osteoporotic vertebral compressive fractures.
