摘 要: | 目的 研究糖基化终产物及其受体在妊娠期糖尿病和非妊娠期糖尿病孕妇中的水平差异,以及糖基化终产物及其受体水平对围产儿结局的影响.方法 选取在我院产前检查并生产的妊娠期糖尿病孕妇90例作为研究组,选取同孕期无妊娠期糖尿病的孕妇90名作为对照组.比较两组的糖基化终产物水平,并对比研究组治疗前后与同孕期对照组的糖化血红蛋白和糖基化终产物水平,检测并比较两组胎盘组织中糖基化终产物受体的水平,采用logistic回归分析围生儿结局与其糖基化受体等的关系.结果 研究组的糖基化终产物水平[(50.42±16.44)ng/L]水平明显高于对照组[(30.23±11.22)ng/L],且在治疗后差异仍然存在[(44.33±11.54) ng/L vs(37.42±9.42)ng/L];而采用West-ern印迹法测量的研究组胎盘组织中糖基化终产物受体水平(0.998±0.076)也明显高于对照组(0.475±0.013).回归分析显示,高糖基化终产物水平是影响围产儿结局的一个危险因子(OR=7.342,P<0.01,95%CI=3.691-10.993).结论 高糖基化终产物水平是影响妊娠期糖尿病围生儿结局的一个不利因素,糖基化终产物水平可作为其围生儿结局异常的一个重要检测指标.

Abstract: | Objective To explore the relationship between serum advanced glycation end products level in gestation diabetic patients and their perinatal outcomes,and to observe the RAGE protein expression in placenta.Methods 90 pregnant women with gestational diabetes were selected as research group,90 normal pregnant women as control group.The glycation end product,glycosylated hemoglobin and glycosylation receptor were measured.Results The glycosylation end products of study group (50.42± 16.44 ng/L) was significantly higher than that in the control group (30.23 ± 11.22ng/L).The differences after treatment still existed (44.33 ± 11.54 ng/L VS 37.42 ± 9.42 ng/L).The glycation end products receptor level (0.998±0.076) was also significantly higher than that in the control group (0.475±0.013).Regression analysis showed high glycosylation end products levels affected perinatal outcome of a risk factor (OR =7.342,P<0.01,95% CI=3.691-10.993).Conclusion The high glycosylation end product is a negative factor to influence gestational diabetes perinatal children.The level of glycosylation end product may be an important index of perinatal children ending abnormal in gestational diabetes mellitus. |
