侧胸皮瓣在颌面部缺损修复中的应用

目的:评价侧胸皮瓣在颌面部缺损修复中的价值。方法:2004～2006年间对17例颌面部软组织缺损患者实施侧胸皮瓣缺损修复术,其中男12例,女5例,平均年龄48.4岁。所有患者均制作单纯侧胸皮瓣或侧胸皮瓣复合肩胛骨肌瓣,游离后进行小血管吻合术,最后修复缺损的术区。结果:除1例患者因支配动脉纤细而出现表皮坏死外,其余患者的皮瓣全部成活,经过3～32个月的随访,外形修复效果较好,供区切口隐蔽、瘢痕小。结论:利用侧胸皮瓣修复颌面部缺损,可以改善患者术后的生存质量,值得临床推广。

The application of lateral thoracic flap in reconstruction of maxillofacial defects

PURPOSE:This study aims to evaluate the usefulness of lateral thoracic flap in reconstructing maxillofacial defects to improve the quality of life of the patients.METHODS:17 cases with soft tissue defects in the maxillofacial region,caused by various disease,were collected during 2003-2006.The age of these patients ranged from 29-71 years(48.4 in average) ,the ratio of male to female was 12:5.The lateral thoracic flap or combined with the scapular flap were applied to reconstruct the defect of soft tissue and/or bone tissue.RESULTS:All the flaps survived except one case which presented necrosis of the epidermis resulting from the too fine supplying artery.After 3-32 months' follow up,the morphology of the reconstructed region was satisfied,and the scar in the donate site was unconspicuous.CONCLUSIONS:The application of thoracic flap is a better choice to improve quality of life of the patients compared with other free flaps traditionally used in clinic,and this donor site is recommended.
