咪达唑仑联合盐酸屈他维林应用于结肠镜检查的疗效观察及护理

目的 探讨咪达唑仑联合盐酸屈他维林应用于结肠镜检查的效果、安全性及护理要点.方法 将100例行结肠镜检查的患者随机分为2组：观察组（50例）静脉注射咪达唑仑与盐酸屈他维林后行结肠镜检查;对照组（50例）按常规进行结肠镜检查.比较2组结肠镜检查的成功率、检查中患者的感受、入镜时间,观察其清醒时间及检查前、中、后血压,心率和血氧饱和度的变化.结果 观察组患者检查成功率为98%,无一例有痛苦反应,入镜时间7.2 min;而对照组检查成功率为92%,所有病例都存在不同程度的疼痛反应,入镜时间10.6 min（P＜0.01）.观察组检查前、中、后收缩压、心率无明显变化,对照组变化明显（P＜0.01）;2组检查前、中、后的舒张压和血氧饱和度无显著性差异（P＞0.05）.观察组患者多于30 min内清醒.结论 咪达唑仑联合盐酸屈他维林应用于结肠镜检查,可有效减轻患者的痛苦、遗忘不良记忆,缩短操作时间,是一种安全、有效的方法;而严密的用药护理是确保药物疗效的关键.

Investigation and nursing on colonoscopy with midazolam and drotaverine hydrochloride

Objective To investigate the effect,security and nursing points of colonoscopy performed with midazolam and drotaverine hydrochloride.Methods 100 patients were randomly divided into the observation group(n=50) and the control group(n=50).The colonoscopy was conducted in the observation group after the intravenous injection of midazolam and drotaverine hydrochloride.The control group was given colonoscopy routinely.The rate of success,the feeling of the patients during examination and the operation duration were compared between the two groups.The level-headed time and the change of blood pressure before,during and after the examination,and the change of pulse rate and blood oxygen saturation were observed.Results The successful rate of colonoscopy,in the observation group and the control group,was 98% and 92%,respectively.There was a significant difference between the two groups(P<0.05).The observation group had no complains of pains and the operation duration is 7.2 minutes.The control group had different degrees of pain reaction and the operation duration is 10.6 minutes(P<0.01).There was no significant difference in diastolic pressure and blood oxygen saturation in every stage during examination in the two groups(P>0.05).There was no significant change of systolic pressure and heart rate before,during and after examination in the observation group.But,there was a significant change of systolic pressure and heart rate in the control group(P<0.01).Conclusions With midazolam and drotaverine hydrochloride,colonoscopy can effectively mitigate the sufferings of patients,forget the bad memories,and shorten the operation duration.This is a safe and effective method.The key for ensuring the effect of drugs is to keep the nursing of drug treatment strictly.
