EB病毒抗体检测对儿童传染性单核细胞增多症的诊断价值

摘    要:目的对传染性单核细胞增多症(IM)的三种实验室诊断方法进行比较,以寻求一种对IM快速诊断的实验方法。方法对162例临床诊断为IM患儿的血常规、异型淋巴细胞分类计数(AL)、血清嗜异性抗体(HA)及EB病毒抗体(EBV-CA-IgM和EBV-CA-IgG)的敏感性进行比较,分析三种实验室诊断方法正确识别IM的能力。结果 IM患儿外周血AL阳性率41.4%,HA阳性率43.8%,EBV-VCA-IgM阳性率87.7%,EBV-CA-IgG阳性率59.9%,EBV-CA-IgM诊断IM敏感性显著高于AL与HA(2χ分别为75.83,69.08,P〈0.01)。结论 EBV-CA抗体检测对于早期诊断IM具有十分重要的意义,而且方法简便快速,适宜在基层医院开展。

Abstract:Objective To compare three Methods of laboratory diagnosis of infectious mononucleosis(IM) to explore a method for rapid diagnosis of infectious mononucleosis.Methods We compared the Results of routine blood test,atypical lymphocyte(AL),heterophile antibody(AH) and EB virus antibody(EBV-CA-IgM and EBV-CA-IgG) of 162 cases of IM to analyze the capacity of three Methods of laboratory diagnosis for identifying IM.Results In the children with IM,the incidence of AL in peripheral blood was 41.4%,the incidence of HA was 43.8%,the incidence of EBV-VCA-IgM was 87.7%,and the incidence of EBV-CA-IgG was 59.9%.The sensitivity of EBV-CA-IgM was significantly higher than AL(x2=75.83,P0.01)and HA(x2=69.08,P0.01) EBV-CA-IgG in diagnosis of IM.Conclusion Detection of EB virus antibody is of great importance for early diagnosis of IM.It is fast and convenient,which is suitable for the basic hospitals.
