关节镜下微骨折术与Pridie钻孔术修复膝关节软骨全层损伤疗效对比研究

摘    要:目的:比较关节镜下微骨折术与Pridie钻孔术在膝关节软骨全层损伤修复治疗中的疗效。方法选取2013年7月至2015年1月我院骨科收治的134例膝关节软骨全层损伤患者,以随机数表法分为两组,分别给予关节镜下微骨折术(M组,n=67)与Pridie钻孔术(P组,n=67)治疗,比较两组患者的临床疗效。结果 M组和P组患者术后6个月的Lysholm评分分别为(88.7±11.5)分与(81.2±11.2)分,均明显高于术前的(36.8±11.2)分与(37.3±10.7)分,差异均有显著统计学意义(P<0.01),且治疗后M组明显高于P组,差异均有显著统计学意义(P<0.01);M组和P组患者优良率分别为94.0%(63/67)和83.6%(56/67),差异无统计学意义(P>0.05);两组术后均未发现明显并发症与合并症。结论关节镜下微骨折术与Pridie钻孔术均能够使膝关节软骨全层损伤患者获得较为满意的软骨修复及膝关节功能恢复,但微骨折术操作更为简便,热损伤更小,术后血凝块粘附更好,安全性更高。

Comparison of treatment effects between microfracture and Pridie drilling technique with arthroscopic in patients with full-thickness chondral defects in the knee
Abstract:Objective To discuss the clinical effects of microfracture and Pridie drilling technique with ar-throscopic in patients with full-thickness chondral defects in the knee. Methods A total of 134 patients with full-thick-ness chondral defects in the knee, who admitted to Department of Orthopedics of our hospital from July 2013 to January 2015, were randomly divided into the M group (n=67) and the P group (n=67). The M group was treated by microfrac-ture with arthroscopic, and the P group was treated by Pridie drilling technique with arthroscopic. Then the clinical ef-fects between the two groups were contrasted. Results After six months of treatment, the Lysholm scores of M group and P group (88.7 ± 11.5) and (81.2 ± 11.2), respectively] were significantly higher than pre-operation (36.8 ± 11.2) and (37.3 ± 10.7), respectively], P<0.01. Moreover, Lysholm score of M group was significantly higher than P group (P<0.051). There was no significant difference between M group and P group in the excellent rate (94.0% vs 83.6%), P>0.05. There was no complication in the two groups. Conclusion Arthroscopic microfracture surgery and Pridie drilling technique both can obtain satisfactory cartilage repair and recovery of knee joint function in patients with full-thickness chondral defects in the knee, but microfracture surgery has the advantages of more convenient, less thermal damage, bet-ter postoperative blood clot adhesion, and higher safety.
