HPV18型与早期子宫颈腺癌患者预后的相关性

目的：探讨人乳头状瘤病毒(HPV)18型与早期子宫颈腺癌患者预后的相关性。方法选取2000年1月至2011年1月琼海市人民医院收治的110例子宫颈腺癌患者作为研究对象，采用PCR法进行HPV基因型检测，比较HPV18型感染与患者预后的关系。结果110例宫颈腺癌患者中HPV 18型阳性者66例，占60.00%。HPV 18型阳性者的淋巴结转移率(27.27%)、脉管瘤栓率(24.24%)明显高于阴性者(6.82%，4.85%，P<0.05)。HPV 18型阳性患者与阴性患者的5年无瘤生存率(86.36%vs 86.36%)、5年总生存率(87.88%vs 90.91%)、无瘤生存时间[(63.54±10.54)个月vs (65.01±12.41)个月]、总生存时间[(73.87±12.66)个月vs (75.11±13.68)个月]比较差异均无统计学意义(P>0.05)。结论 HPV18型感染与早期子宫颈腺癌患者的癌细胞转移有关，但其与患者的总生存时间、无瘤生存时间无明显相关性。

Correlation between HPV Type 18 and prognosis of patients with early adenocarcinoma of the cervix

Objective To discuss the correlation between the Human Papilloma Virus (HPV) Type 18 and early prognosis of patients with adenocarcinoma of the cervix. Methods One hundred and ten patients with adenocarcinoma of the cervix treated in People's Hospital of Qionghai City from Jan. 2000 to Jan. 2011 were selected as the study sub-jects. PCR was used to detect HPV genotype, and the correlation between HPV Type 18 and prognosis were analyzed. Results Among the 110 patients, 66 (60.00%) were detected with positive HPV Type 18. The lymph node metastasis, and vascular invasion rate of patients with positive HPV Type 18 (27.27%, 24.24%) were significantly higher than those of negative patients (6.82%, 4.85%), P<0.05. There were no significant differences in 5-year disease-free survival rate (86.36%vs 86.36%), 5-year overall survival rate (87.88%vs 90.91%), disease-free survival time [(63.54±10.54) months vs (65.01 ± 12.41) months] and overall survival time [(73.87 ± 12.66) months vs (75.11 ± 13.68) months] between the pa-tients with positive HPV Type 18 and the negative patients (P>0.05). Conclusion The genotype of HPV Type 18 infec-tion is correlated with cancer cell metastasis of patients with adenocarcinoma of the cervix, but there is no obvious corre-lation between the overall survival and disease-free survival of the patients.
