摘    要:目的:探讨脑梗死后应激性高血糖对患者自主活动能力恢复的影响。方法选取2014年3~9月期间本医院神经内科收治的脑梗死患者78例,入院次日抽取静脉血检测空腹血糖(FBG),据血糖水平将患者分为高血糖组(FBG≥6.1 mmol/L,n=32)和正常血糖组(FBG<6.1 mmol/L,n=46),确诊后均按照急性脑梗死治疗方案给予常规治疗,应用Barthel指数作为评价指标,观察并比较两组患者治疗前后自主活动能力的恢复情况。结果入院时高血糖组Barthel指数评分为(46.1±12.4)分,正常血糖组为(45.6±13.1)分,组间比较差异无统计学意义(P>0.05);治疗7 d、14 d,血糖升高组患者Barthel指数评分分别为(53.1±14.9)分、(60.4±15.3)分,明显低于血糖正常组的(68.2±14.9)分和(80.3±17.9)分,差异均有统计学意义(P<0.05)。结论应激性高血糖者神经功能恢复差,FBG可作为评价病情及预后的指标。

Influence of stress hyperglycemia after cerebral infarction on recovery of daily living activity in the patients
Abstract:Objective To analyze the influence of stress hyperglycemia after cerebral infarction on recovery of daily living activity in the patients. Methods Seventy-eight patients with cerebral infarction were selected in Depart-ment of Neurology in our hospital from March to September in 2014, who were detected for fasting blood-glucose (FBG) in the next day after admission. According to the FBG level, the patients were divided into patients with stress hy-perglycemia group (FBG≥6.1 mmol/L, n=32) and euglycemia group (FBG<6.1 mmol/L, n=46). After diagnosis, all pa-tients were given routine treatment according to the treatment of acute cerebral infarction. Barthel index was used as evaluation index. The recovery of daily living activity of the two groups of patients before and after the treatment were observed and compared. Results On admission, the Barthel index score of hyperglycemia group was (46.1±12.4), and the score of euglycemia group was (45.6 ± 13.1), with no significant difference between the two groups (P>0.05). The Barthel index score in hyperglycemia group after treatment for 7 days and 14 days were significantly lower than those in euglycemia group [(53.1±14.9) vs (68.2±14.9), (60.4±15.3) vs (80.3±17.9), P<0.05]. Conclusion Neural functional re-covery is poor in patients with stress hyperglycemia, and the FBG can be used as an index for evaluating the severity and prognosis of the disease.
