472例手足口病临床分析

目的：分析本地区手足口病临床特征。方法：采用回顾性调查分析的方法对本院2009年5月～2010年4月感染科收治的472例手足口病患者的临床资料进行研究分析。结果：472例患者中,男271例,女201例,5岁以下者占87%,发病季节主要集中于4～7月份,共收治手足口病患者265例,占56.1%。主要表现依次为皮疹、发热等。3例出现肺损害,15例出现神经系统受累,7例出现肝功能损害,287例出现心肌损害,12例并发肺炎,其中重症患者24例,占5.1%。总肠道病毒阳性率为54.2%（65/120）,其中EV71阳性率为38.5%（25/120）,CoxAl6阳性率为55.4%（36/120）,两者均阳性6.2%（4/120）。24例重症患者中,7例是CoxA16感染,17例是EV71感染。448例普通患儿痊愈出院,24例重症患儿中,死亡1例,其余痊愈,但病程较长。结论：手足口病多发于5岁以下儿童,发病季节主要集中于4～7月份,以EV71和CoxA16病毒感染为主。应早期诊断,及时治疗,防止危重并发症的出现,从而降低手足口病的病死率。

Clinical analysis of 472 cases infected with hand-foot-mouth disease (HFMD)

Objective：To investigate the clinical characteristics of hand-foot-mouth disease（HFMD） in the region.Methods：To analyze the clinical characteristics of 472 HFMD cases collected in our hospital from May 2009 to April 2010 retrospectively.Results：271 of 472 cases were male,210 cases were female,with 87% cases aged less 5.Onset seasons were April to July.3 cases developed lung injury,15 cases with encephalitis,7 cases with liver injury,287 cases with myocardial injury,12 cases with pneumonia,24（5.1%） of 472 cases were severe.Enterovirus was found in 54.2%（65/472） of cases,specifically EV71（38.5%,n=25）,CA16（55.4%,n=36）,EV71 and CA16（6.2%,n=4）.7 of 24 severe cases were infected with CA16,17 cases with EV 71.448 normal cases got recovery and 24 serve cases in which one child dead,the others got recovery with a long course of disease.Conclusion：Most cases of HFMD occurred in children are less than 5 years old.The study demonstrate that EV71 and CA16 are the most prevalent present cause of HFMD in Taizhou.To decrease the fatality of HFMD,HFMD should be diagnosed early and treated timely and severe complications should be prevented.
