手术室环境温度对老年人手术期间核心温度变化的影响

目的:探讨手术室环境温度和老年病人全麻后上腹部手术期间核心温度变化的关系。方法:100例年龄大于65岁、在全身麻醉联合硬膜外麻醉下择期行上腹部手术的病人。根据手术室环境温度设置的不同,随机将病人分为4组:Ⅰ组18～20℃,Ⅱ组～22℃,Ⅲ组～24℃,Ⅳ组～26℃。术中持续监测并记录核心温度(食管温度)。分析核心温度改变同手术室环境温度的相关性。结果:4组病人麻醉诱导后体温均开始下降。在诱导后60min下降幅度最大;在诱导后60min、120min以及术毕,Ⅰ组同Ⅲ、Ⅳ组比较差异有显著意义(P<0.05),Ⅱ组同Ⅳ组比较差异有显著意义(P<0.05);Ⅱ组同Ⅲ组比较在诱导后60min、120min两个时间点,差异有显著意义(P<0.05)。术毕核心体温小于35℃的发生率:Ⅰ组显著高于Ⅱ、Ⅲ、Ⅳ组(P<0.01),Ⅱ组显著高于Ⅳ组(P<0.01)。在诱导后60min和术毕的体温下降幅度与环境温度存在回归关系。结论:环境温度直接影响老年病人全麻后上腹部手术期间的核心温度变化。
