脾脏肿瘤治疗经验

目的　总结研究脾脏肿瘤的诊治经验。方法　回顾分析我院 16年来 16例脾脏肿瘤的病理类型、对其的诊断和治疗以及患者的生存期限。结果　 16例脾脏肿瘤中 ,良性肿瘤占 9例 ,恶性肿瘤占 7例。良性肿瘤主要为脾血管瘤、错构瘤、淋巴管瘤及炎性假瘤 ;恶性肿瘤主要为恶性淋巴瘤、血管肉瘤、纤维肉瘤及胃癌脾脏转移癌。病变早期无明显症状和体征 ,诊断主要依靠体检和影像学检查。治疗主要是脾切除术 ,恶性肿瘤同时应用化疗和放疗。良性肿瘤对生存期无明显影响 ,恶性肿瘤的生存期限较短。结论　早期诊断和早期治疗是提高恶性肿瘤生存率和延长生存时间的关键。完善的体检和准确的影像学检查是提高脾脏肿瘤检出率的主要手段。

Treatment of splenic tumors-A report of 16 cases

Objective To summarize the treatment experience of splenic tumors.Methods A retrospective study was performed on 16 cases of splenic tumors in our hospital in 16 years.The pathological types, diagnosis, treatment of splenic tumors and the survival time of the patients with splenic tumors were analyzed.Results Nine out of the 16 cases were diagnosed as having benign splenic tumor and the remaining malignant.Benign splenic tumors included hemangioma, inflammatory pseudotumors, lymphangioma and hamartoma.Malignant splenic tumors included Fibrosarcoma, hemangiosarcoma, lymphoma and metastatic tumors.There were no obvious symptoms and signs in early periods.Physical examination and image examinations were the main methods for splenic tumors.All patients were treated by splenectomy, whereas chemotherapy and radiotherapy were performed on malignant splenic tumors.Conclusion Early diagnosis and treatment were important for prolonging the survival time.Perfect physical examination and exact image examination were the main means of diagnosis for splenic tumors.
