慢性肺源性心脏病急性期合并肺性脑病动脉血液气体分析

摘    要:为探讨慢性肺源性心脏病急性期合并肺性脑病时血气变化情况,将24例肺性脑病患者和74例Ⅱ型呼吸衰竭患者作对比研究。结果:肺脑组PaCO2升高、pH降低较呼衰组更明显(P<0.01),肺脑组PaO2比呼衰组低(P<0.05);肺脑组以呼酸合并代酸最常见。提示:pH值与肺性脑病呈负相关(γ=-0.665),PaO2与肺性脑病呈正相关(γ=0.751)。

Abstract:Objective To investigate the changes in arterial blood gases in the acute stage of chronic cor pulmonale complicated with pulmonary encephalopath.Methods:A control study was made in 24 of such patients and 74 with respiratory insufficiency but no encephalophthy.Result:Blood pH Levels decreased in group with pulmonary encephalopath,meanwhile increase of PaCO2 and decrease of PaO2 in group with pulmonary encephalopath were significate compared with group without pulmonary encephalopath(p<0.01 ̄0.05).There is a high risk of pulmonary encephalopath when respiratory acidosis combined with metabolic acidosis,and solitery respiratory acidosis combined with metabolic acidosis,and solitery respilatory acidosis just follows it.Conclusion:There is a negative relation between blood pH values and pulmonary encephalopathl(γ=-0.655),but a positive relation between PaCO2 and pulmunary encephalopath(γ=0.751).
