胰岛素联合阿仑膦酸钠对2型糖尿病骨质疏松症患者骨代谢的影响

目的研究胰岛素联合阿仑膦酸钠对2型糖尿病骨质疏松患者骨代谢的影响。方法选取2016年8月~2019年8月随机纳入的60例确诊为2型糖尿病合并骨质疏松患者作为观察对象。随机数字法将患者分为对照组和观察组。对照组患者以单一胰岛素治疗,观察组患者以胰岛素联合阿仑膦酸钠治疗,对比两组患者治疗前后的骨代谢指标[BMD(骨密度指数)、BGP(骨钙素)、BAP(血清骨特异型碱性磷酸酶)、TRAP-5b(人抗酒石酸酸性磷酸酶)],统计两组患者在治疗期间不良反应发生情况。结果治疗前,两组患者之间的骨代谢指标差异无统计学意义(P>0.05),治疗后均得到了改善,观察组改善效果更显著,与对照组比较差异有统计学意义(P<0.05);对照组不良反应发生率为6.67%,观察组发生率为20.00%,差异无统计学意义(P>0.05)。结论胰岛素联合阿仑膦酸钠治疗2型糖尿病骨质疏松疗效显著,对于患者骨代谢情况的改善有着积极意义。

Effects of insulin and alendronate on bone metabolism in patients with type 2 diabetic osteoporosis and its application significance

Objective To study the effect of insulin and alendronate on bone metabolism in patients with type 2 diabetic osteoporosis.Methods The study started and ended in August 2016 and August 2019,respectively.During this period,60 patients were randomly included as observation objects.All patients were treated in our hospital during the study period.Treatment methods were divided into groups,and patients were numbered by random number method.Singular patients were included in the control group,and even patients were included in the observation group.Control patients with single insulin treatment,observation group of patients with insulin joint Allen phosphonic acid sodium treatment,compared two groups of patients before and after treatment of bone metabolism index[BMD(bone mineral density index),BGP(osteocalcin),BAP(serum bone alkaline phosphatase(alien),the TRAP-5 b tartrate resistant acid phosphatase(people)],statistics of two groups of patients with adverse reactions during treatment happening.Results Before treatment,there was no statistical difference in bone metabolism indexes between the two groups(P>0.05),and both groups were improved after treatment.The improvement effect was not obvious in the control group,but significant in the observation group,and the bone metabolism related indexes in the observation group were significantly better than those in the control group(P<0.05).2 patients in the control group had adverse reactions,with an incidence of 6.67%;6 patients in the observation group had adverse reactions,with an incidence of 20%.The observation group was slightly higher than the control group,but there was no statistical difference(P>0.05).Conclusion For the clinical treatment of patients with type 2 diabetes and osteoporosis,insulin combined with alenfosonate has a good effect and positive significance for the improvement of bone metabolism in patients.
