中国3个城市社区人群脑卒中死亡及其类型分布特征

目的　分析我国脑卒中死亡类型分布特征。方法　采用设平行对照的社区人群脑卒中危险因素综合干预研究 ,在长沙、北京和上海 3个城市社区 30万人群中 ,对 14 0 7例脑卒中死亡病例进行分型诊断。结果　脑卒中死亡病例中因出血性卒中、缺血性卒中和难分类型卒中死亡的分别占 6 0 .6 %、38.7%和 0 .7%。各年龄组出血性卒中死亡比例均在 5 0 %以上 ,出血性卒中在起病后 2 8d内 (特别是 2 4h内 )死亡比例显著高于缺血性卒中 (P <0 .0 5 )。结论　出血性卒中仍是卒中死亡的主要原因。以防治高血压为重点的脑血管病综合措施对降低脑卒中的死亡已起积极作用

Stroke mortality and distribution characteristics of types of stroke in three urban areas in China

Objective To analyze the stroke mortality and the distribution characteristics of types of stroke in China.Methods A parallel controlled comprehensive prevention study of risk factors of stroke in community was conducted in 300?000 population of 3 urban communities in Changsha,Beijing and Shanghai,and the typing diagnoses of 1?407 cases of stroke fatal at their first attack were studied.Results Hemorrhagic stroke mortality accounted for 60.6%,ischemic 38.7%,and unclassified 0.7%.The proportion of hemorrhagic stroke was more than 50 percent of all aged mortal cases.The mortality rate of hemorrhagic stroke within 28 days,especially within 24 hours was significantly higher than that of the ischemic stroke(P<0.05).Conclusion Hemorrhagic stroke was the main cause of stroke mortality.The cerebrovascular disease comprehensive prevention,mainly prevention of hypertension,should decrease the stroke mortality.
