适形放疗联合甲地孕酮治疗原发性肝癌伴门静脉瘤栓的临床研究

目的探讨适形放疗联合甲地孕酮治疗原发性肝癌伴门静脉主干瘤栓的临床疗效及毒副反应。方法治疗组25例给予适形放疗联合甲地孕酮,对照组25例单纯给予适形放疗,观察两组疗效、不良反应发生率及生活质量情况。结果治疗组的近期疗效高于对照组,但两组相比无统计学意义。治疗组白细胞下降的发生率低于对照组,生活质量较对照组明显提高,两组比较有统计学意义。结论适形放疗可改善原发性肝癌局控率及生存率,有重要的临床应用价值。甲地孕酮可改善患者的提高生活质量、保护骨髓,提高患者放疗耐受性。

The clinical observation for conformal therapy and megestrolacetate treatment of liver cancers with portal vein tumor thrombosis

Objective To detect the clinical efficacy of conformal radiotherapy and megestrolacetate treating primary liver cancer with portal vein tumor thrombosis.Methods 50 liver cancers with portal vein tumor thrombosiswere divided into trial group and contral group randomizly.The trial group were treated with conformal radiotherapy and megestrolacetate.The control group received conformal radiotherapy only.The changes of effective rate,bone marrow function,toxic and adverse reaction,quality of life in patients of two groups were observed.Results The response rate of the trial group was a little bit higher than the control group.The incidence rate of WBC descent was lower in the treatment group than in the control group.The treatment group excelled significantly control group in quality of life,and there was a significant difference between the two groups.Conclusions Conformal radiotherapy can improve local control rate and survival.Megestrolacetate is an effective and safe drug in improving the quality of life and protecting bone marrow of patients treated with radiotherapy.
