食管癌肉瘤11例

目的 依据食管癌肉瘤的临床特点、病理特点，探讨其可能的组织学来源。方法对11例食管癌肉瘤病人的临床资料进行回顾性研究。结果食管癌肉瘤多为息肉样或蕈伞样腔内生长，一般有蒂与食管壁相连，个别呈浸润性生长。光镜下肉瘤与癌两种成分共存，瘤体多以肉瘤成分为主，癌多局限于蒂的基底部，且多为早期鳞癌。11例患无1例肿瘤外侵。淋巴转移率为9.1％，手术切除率100％，术后1、3、5年存活率分别为90．9％、80％、50％。结论食管癌肉瘤是一种侵袭性差，淋巴转移率低，预后较佳的肿瘤。手术切除是其首选和主要治疗手段．其组织学来源可能是食管粘膜的多潜能原始干细胞。

The Sarcoma of Esophagus Cancer in 11 Cases

Objective To explore the possibility of the source of its history according to the clinical and pathological character of the cancer of the esophagus.Methods 11 cases that had the cancer of the esophagus were retrospectively studied.Results The cancer of the esophagus was mainly intracavitory growth which appeared polyp.And it often connected the wall of esophagus with pedicle.Only one or two cases were infiltrative growth. Under the microscope, we could observe that the sarcoma and cancer exist together.The main content of the tumor was sarcoma. The cancer was generally confined to the basis of the pedicle and most of cases were early stage carcinoma squamosum. None of them belonged to the invasive tumor. The ratio of lymphogenous metastasis and excision was 9.1% and 100%, and the survival ratio (1,3 and 5 years after operation) was 90.9%, 80% and 50%.Conclusion The sarcoma of the cancer of the esophagus is a kind of tumor, which is weak invasiness, low lymphogenous metastasis ratio and favorable prognosis. It is believed that the excision is the first choice and the main therapeutic method. And the possibility of its source of histology may be the multi-potential stem cell which lies in the mucous membrane of the esophagus.
