糖皮质激素在老年结直肠癌并急性肠梗阻急诊一期切除中的应用

目的探讨糖皮质激素在老年结直肠癌并急性肠梗阻急诊一期切除术后应用的可行性。方法 81例老年结直肠癌并急性肠梗阻患者行一期切除术后,随机分为实验组41例(术后应用地塞米松10mg/d,2～3d)、对照组40例(术后常规处理),观察吻合口并发症及不良反应。结果对照组与实验组的吻合口瘘并发症差异无统计学意义(P>0.05);排气时间、恶心呕吐、肠梗阻的差异有统计学意义(P<0.05),实验组优于对照组。结论老年结直肠癌并急性肠梗阻急诊行一期切除吻合后应用糖皮质激素,可加快患者术后康复。

Application of glucocorticosteroid in elderly patients with colorectal cancer and acute obstruction after emergency one-stage resection

Objective To investigate the feasibility of applying glucocorticosteroid in elderly patients with colorectal cancer and acute obstruction after emergency one-stage resection.Methods Eighty-one patients with colorectal cancer and acute obstruction were randomly divided into two groups.The study group(n=41) received 10 mg/d glucocorticosteroid for 2～3 days,and the control group(n=40) received routine treatment.Complications of anastomotic stoma and other adverse reactions were observed.Results There was no statistically significant difference in complications of anastomotic stoma between the two groups(P >0.05),while statistically significant difference was found in the mean farting time,the incidence of nausea and obstruction between the two groups(P<0.05).Conclusion Administration of Glucocorticosteroid can promote the recovery after emergency one-stage operation in elderly patients with colorectal cancer and acute obstruction.
