急诊内科昏迷患者220例诊治体会

目的探讨急诊内科昏迷患者的病因构成和早期诊断处理方法,以降低病死率。方法对我院急诊内科2010年1月至2011年6月收治的220例内科昏迷患者的临床资料进行回顾性分析,分析昏迷患者的病因构成和总结早期诊断处理的经验体会。结果本组患者昏迷原因比例最高为酒精中毒,达22.27%(49/220),其次为脑出血,比例为18.18%(40/220);病死率最高的是溺水和脑出血,比例分别为50.00%和45.00%。抢救之后,好转184例(83.64%),死亡36例(16.36%)。结论恰当的早期处理、尽快地做出病因诊断和病因治疗、保护好患者的多脏器的功能、预防感染是抢救昏迷患者的重要措施,对提高抢救成功率、降低病死率具有重要临床意义。

Medical diagnosis and treatment of 220 cases of coma in the Emergency Department

Objective To investigate the medical cause of the unconscious patients, and to explore early diagnosis and treatment methods to reduce the mortality. Methods The clinical data of 220 patients of medical coma from January 2010 to June 2011 was retrospectively analyzed to explore the cause, and the early diagnosis as well as the treatment of the disease. Results The cause of the coma was mainly alcoholism, accounting for 22.27% (49/220), followed by cerebral hemorrhage (18.18%, 40/220). Drowning and cerebral hemorrhage resulted in the highest mortality, 50.00% and 45.00% respectively. After the rescue, 184 patients were improved (83.64%), and 36 died (16.36%). Conclusion To adopt an appropriate early treatment, make a diagnosis of the cause as soon as possible, protect the patient’s multiple organ function and prevent infection were important measures for the rescue of the unconscious patient, which are of great importance to improve the success rate and lower the mortality.
