药物致老年急性肾功能衰竭临床研究

目的　研究药物所致老年急性肾功能衰竭 (ARF)的临床特点及其预后。方法　分析我院近 10a来收治的 78例药物导致的ARF患者的临床特点及其预后。结果　老年组 4 4例 ,少尿型ARF为 2 7例 ,少尿时间为 14d ,38例需要透析 (占86 36 % ) ;中青年组 34例 ,少尿型ARF为 14例 ,少尿时间为 8d ,2 4例需要透析 (占 70 5 8% ) ,两组差异显著 (P <0 0 5 )。两组间肾脏大小、Scr、BUN、K 、Hb均无统计学差异。老年组中 19例肾功能恢复正常 (占 4 3 18% ) ,2 1例转为慢性肾功能衰竭 ,9例需要透析 (占 2 0 5 3% ) ,4例死亡 (占 10 2 % )。中青年组中 2 0例肾功能恢复正常 (占 5 8 82 % ) ,12例转为慢性肾功能衰竭 ,3例需要透析 (占 8 83% ) ,1例死亡 (占 2 94 % )。结论　患者的年龄、基础疾病、少尿期长短和药物的种类是影响药物导致的急性肾功能衰竭程度和患者存活率的重要因素

Clinical study of old patient acute renal failure caused by medicine

Objective We studied the clinical feature and prognosis of old patients acute renal failure caused by medicine Method We analyzed the clinical feature and prognosis of 78 patients with acute renal failure caused by medicine who were admitted to our hospital in recent ten years Result In old group, 27 patients had acute renal failure among 44 uropenia patients,uropenia time was 14 days 38 patients needed dialysis(toal 86 36%) In youth group, 14 patients had acute renal failure among 34 uropenia patients, uropenia time was 8 days, 24 patients needed dialysis(total 70 58%) They were significantly different between two groups( P <0 05),there was no statistical difference in kidney size and test of Scr,BUN and electrolyte and hemoglobin between two groups In old group, 19 patients' renal function recovered(total 43 18%),21 patients developed chronical renal failure,9 patients needed dialysis(total 20 53%),four patients died(total 10 2%) ln youth group, 20 patients' renal function recovered(total 58 82%),12 patients developed chronical renal failure,3 patients needed dialysis(total 8 83%),one patient died(total 2 94%) Conclusion The patient's age and elementary disease and uropenia time duration,variety of medicine are important factors which affect acute renal failure sickness and patients viable rates
