胫骨下端Pilon骨折的疗效观察

目的：回顾胫骨下端Pilon骨折治疗病例,探讨胫骨下端Pilon骨折的治疗方法。方法：对2000年7月—2007年6月24例Ruedi-Allgower型胫骨下端Pilon骨折患者给予有限内固定结合外固定治疗,同时术后给予必要的对症治疗与科学的康复锻炼。结果：随访14～32个月,按照Mazur评分系统评估疗效,优10例,良12例,中1例,差1例,优良率达91.7%。1例发生伤口感染,无深部感染;1例胫前皮肤坏死;2例外固定器针眼部皮肤感染;1例关节退行性变。结论：有限内固定结合外固定治疗Ruedi-Allgower型胫骨下端Pilon骨折,可以减少并发症,取得良好治疗效果。关键词胫骨下端骨折;内固定;外固定

The observation of therapeutic effect of Pilon fractures type lower end of tibia

Objective：To summarize retrospectively the clinical therapeutic effect of pilon fractures type lower end of tibia.Methods：From July 2000 to June 2007,24 patients with Ruedi-Allgower Type Ⅲ pilon fractures were treated with limited internal fixation combined with external fixation,patients accepted necessary treatment to symptomatic and rehabilitation exercise after surgery.Results：All patients were available at the final follow-up,the mean duration was 21 months（range 14～32months）.At the last examination,the curative effect revealed excellent in 10 patients,good in 12 patients,fair in 1 patients and poor in 1 patients according to mazur function assessment system.Conclusion：Limited internal fixation combined with external fixation can acquire good therapeutic effect and remarkably reduce complication in Ruedi-Allgower Type Ⅲ pilon fractures.
