| 摘 要： | 目的：评价替比夫定治疗慢性乙型肝炎48周的疗效及其影响因素。方法：采用替比夫定600 mg日一次口服治疗60例慢性乙型肝炎患者48周,对治疗前后丙氨酸氨基转移酶（alanine transarninase,ALT）、乙肝病毒DNA（hepatitis BVirus DNA）、HBeAg转阴情况进行比较。同时根据患者治疗4周、12周和24周的HBVDNA来预测影响疗效的因素。结果：与治疗前相比,替比夫定治疗4周、12周、24周及48周各时间点,ALT明显下降,差异有显著性（P〈0.05）;HB-VDNA PCR检测不到患者百分比及HBeAg转阴患者百分比明显增高,差异有显著性（P〈0.05）。治疗24周时HBVDNA水平低于300拷贝/ml,48周ALT复常率、HBVDNA PCR检测不到患者百分比、HBeAg转阴率明显增高,与HBVDNA水平高于300拷贝/ml者比较,差异有显著性（P〈0.05）。结论：替比夫定能明显抑制HBVDNA复制,使ALT复常,促进HBeAg血清转换,治疗24周的HBVDNA抑制水平可预测治疗48周的疗效。
