瑞芬太尼复合丙泊酚全凭静脉麻醉用于小儿耳鼻喉科手术的临床研究

目的探讨瑞芬太尼、丙泊酚静脉麻醉用于小儿耳鼻喉手术的临床价值。方法 80例4-12岁患儿随机分为瑞芬太尼、丙泊酚组（A组）与芬太尼、丙泊酚和七氟醚组（B组）。麻醉诱导后,A组持续输注丙泊酚和瑞芬太尼;B组持续输注芬太尼、丙泊酚,辅助吸入七氟醚维持麻醉。记录术中循环变化,术后麻醉恢复情况。结果手术开始后、手术结束时B组患儿HR、MAP均高于A组（P〈0.01）;A组患儿自主呼吸恢复时间、睁眼时间、拔管时间、定向力恢复时间明显短于B组（P〈0.01）;而在躁动发生率B组低于A组（P〈0.01）。结论丙泊酚和瑞芬太尼全凭静脉麻醉用于小儿耳鼻喉手术安全有效。

Clinic Observation of Remifentanil Combined with Rropofol in Total Intravenous Anesthesia for Pediatric ENT Surgery

Objective To investigate the effect of remifentanil combined with propofol in total intravenous anesthesia for pediatric ENT surgery. Methods 80 children were randomly divided into two groups,namely group A and group B.The patients in group A received intravenous injection of remifentanil and propofol,and those in group B received fentanyl and sevoflurane.The data of hemodynamics and recovery of anesthesia at the end of operation were observed.Results There was significant difference in HR and MAP at the beginning and end of operation between the two groups（P〈0.01）.A better recovery of postanesthesia,including the time of respiratory recovery,eye oper,extubating and orientation force recovery,was found in group A than that in group B（P〈0.01）.But in group A there was higher rate of restlessness after operation than that in group B（P〈0.05）.Conclusion Remifentanil combined with propofol infusion in intravenous anesthesia is a better way of anesthesia for pediatric ENT surgery.
