CT扫描联合酶联免疫吸附法检测血清AFP-L3含量在PHC诊断中的价值

目的通过CT扫描联合酶联免疫吸附法（ELISA）检测血清甲胎蛋白异质体（AFP-L3）含量,探讨其在原发性肝癌（PHC）诊断中的价值。方法137例（其中106例具CT增强扫描的资料）血清标本来自唐山市人民医院2011年3月至11月间的住院患者,运用ELISA法检测血清中AFP-L3的含量,以AFP-L3〉37.89ng/ml为临界值。137例病例均具有CT或CT增强扫描资料,回顾分析137例患者总共164个PHC病灶的增强扫描经后处理技术得到二维及三维重建图像而诊断,探讨采用血清AFP-L3含量与CT增强扫描及这两种方法联合应用在PHC诊断方面的价值。结果以AFP-L3〉37.89ng/ml为临界值诊断原发性肝癌的灵敏度为83.69%（77/92）,特异度为88.88%（40/45）。联合运用血清AFP-L3含量以及CT扫描两种技术诊断PHC的灵敏度为85.57%、特异度为88.57%。结论联合运用ELISA法检测AFP-L3含量及CT增强扫描两种技术对PHC诊断具有重要的诊断价值,在对血清AFP-L3含量低的PHC患者诊断中行CT增强扫描具有非常重要的意义。

Theclinical value of the combined detection of Alpha-fetoprotein-L3 Detected by Enzyme Linked Immunosorbent Assay and contrast-enhanced CT in the diagnosis of primary hepatic cancer

Objective To detect the content of Alpha-fetoprotein-L3 by Enzyme Linked Immunosorbent Assay and contrast-enhanced CT and investigate the clinical value in the diagnosis of primary hepatic cancer. Methods The serum samples of 137patients with positive AFP treated in our hospital between March and November 2011 were studied. AFP-L3 ELISA detection reagent kit provided by Shanghai Yifeng Biotechnology Limited Company was used to detect AFP-L3 concentration. Using the ELISA to detect the levels of serum AFP-L3 ,Taking AFP-L3 level ≥37.89 ng/mL as diagnostic criteria, 137 cases were with enhanced CT or CT scans,review the scan after the treatment technology of 2D and 3D reconstruction of image analysis in 137 patients with a total of 164 PHC lesions and diagnosis,to explore the content of AFP-L3 in serum and CT enhancement scanning and the combination of the two methods in the diagnosis of PHC value. Results Taking AFP-L3 level ≥37. 89 ng/mL as diagnostic criteria, the sensitivity of AFP-L3 level in PH（2 diagnosis was 83. 69% and the specificity was 88. 88%. The sensitivity of AFP-L3 level and CT scans of two kinds of technique in the diagnosis of PHC was 85.57%,the specificity was 88, 57 %. Conclusion ELISA has a high clinical value in detecting AFP-L3 which is a valuable biomarker, AFP-L3 level and CT scan of two kinds of technique have important value in the diagnosis and prediction of prognosis of PHC.
