股骨颈骨折中医保守疗效分析

目的:股骨颈骨折的中医保守治疗疗效分析。方法:对2005年7月—2013年1月接受中医保守治疗的81例新鲜股骨颈骨折患者进行回顾性研究。81例患者出院后获1~6年(平均3.3年)随访,观察骨折愈合、髋部功能状态、并发症情况及股骨头坏死率,按Harris评分标准进行功能评定。结果:优32例,较好18例,良20例,优良率86.4%,4例患者由于严重内科病,住院期间内转院;1例自动转院;1例患者住院87d骨折端仍未开始生长,故出院,后经随访,患者于出院后不久,因骨折端骨肿瘤过世;1例患者,女74岁,最后因骨不愈合行髋关节置换术;1例患者,89岁,住院期间自然死亡;2例患者住院一周内出院而未进行延续治疗;1例出现股骨头坏死。结论:中医保守治疗股骨颈骨折,疗效可靠,不切开关节囊,不暴露骨折端,对股骨头血液循环干扰小,能够获得满意功能。

Analysis of Chinese Conservative Treatment for Femoral Neck Fracture

Objective ： To analyze the effect of Chinese medicine conservative treatment of femoral neck fracture. Methods： From July 2005 to January 2013,81 patients with fresh femoral neck fracture were retrospectively studied. 81 patients discharged from hospital after received 1-6 years follow-up （ mean 3.3 years ）. We observed fracture healing, hip function, complications, and femoral head necrosis rate, according to Harris scoring criteria for functional assessment. Results ： Excellent ： 32 cases ; good ： 18 cases ; effective： 20; the total effective rate was 86.4%. 4 cases of patients, due to serious medical illness, changed hospital and 1 case made automatic transfer. 1 patient still didn＇t start growing after 87 days and discharged, after the follow-up, the patient soon passed away due to fracture bone tumors. 1 patient,74-year-old female, finally did hip rephmement for bone did not heal. 1 patient, 89 years old, died naturally in hospital. 2 patients discharged a week later and did no continue therapy. 1 case had femoral head necrosis. Conclusion ： Chinese medicine conservative treatment of femoral neck fracture has reliable curative effect without joint capsule incision or exposing fracture end and it has small disturbance to femoral head blood circulation and can obtain satisfactory function.
