后路椎板成形并突出髓核摘除治疗颈椎管狭窄并颈椎间盘突出症

目的 探讨单开门后路减压并突出髓核摘除治疗颈椎管狭窄并颈椎间盘突出症的疗效.方法 1999年8月至2008年8月,采用后路单开门减压并突出髓核摘除治疗19例颈椎管狭窄并颈椎间盘突出症患者,18例获得平均46个月(3-108个月)的随访,观察比较手术前后神经功能(JOA评分)的变化情况.结果 18例患者术后JOA评分(14.22±1.86)分,与术前(6.72±2.42)分比较差异有统计学意义(P<0.01),术后神经功能平均改善77%.结论 后路单开门减压并突出髓核摘除治疗颈椎管狭窄并颈椎间盘突出症安全可靠.

One stage laminoplasty with posterior herniotomy for treating myelopathy caused by cervical stenosis combined with cervical disc herniation

Objective To study the outcome of laminoplasty with posterior herniotomy for treating myelopathy caused by cervical stenosis combined with cervical disc herniation. Method Eighteen patients underwent this procedure were reviewed. The average follow-up period was 46 months. Neurologic function were evaluated with the Japanese Orthopaedic Association (JOA) Scoring System. Preoperative and postoperative neurologic function(JOA) were compared. Results Postoperative JOA score(14. 22±1.86) was significantly higher than preoperative score (6.72 ±2. 42) (P < 0. 01) . The mean JOA score improvement was 77%. Conclusions This technique is safe and effective for treating myelopathy caused by cervical disc herniation combined with cervical stenosis.
