右室Tei指数在慢性肺源性心脏病中的诊断价值

目的应用右室Tei指数测定慢性肺源性心脏病患者右心功能的价值。方法超声观察测量47例临床怀疑慢性肺源性心脏病患者及35例正常人常规超声指标,测量并计算右室Tei指数。超声测量指标包括右房横径、右室横径、右室前壁厚度、肺动脉主干内径、根据三尖瓣返流估测肺动脉收缩压。结果47例患者中16例具有较明确的肺心病超声征象和中度以上肺动脉高压,20例无明显二维超声改变但有不同程度肺动脉高压,另11例患者无肺心病二维超声征象,又无三尖瓣或肺动脉瓣少量返流,多谱勒超声无法估测肺动脉压,超声诊断为正常超声心动图。上述3类患者右室Tei指数与对照组相比差异均有统计学意义。结论Tei指数的测量可为临床提供一定的诊断信息,对治疗、预后有一定的帮助。

The diagnostic value of Tei index for cardiac function in patients with chronic pulmonary heart disease

Objective To determine the value of using the right ventricular Tei index for cardiac function in patients with chronic pulmonary heart disease.Methods 47 clinical cases with suspected chronic pulmonary heart disease and 35 cases with normal ultrasounds as a standard were analyzed by using ultrasound examination for measuring and calculating the right ventricle Tei index.Ultrasound measurement norms included the right atrium horizontal path,the right ventricle horizontal path the right ventricle anterior wall thickness,pulmonary main artery diameter,using tricuspid valve regurgitation to estimate arterial systolic pressure.Results In 47 cases,16 had relatively obvious cardiopulmonary disease ultrasound signs and medium or above pulmonary artery high blood pressure,20 cases with no obvious two-dimensional ultrasound changes but had different degrees of pulmonary artery high blood pressure,ultrasound diagnosis for conventional ultrasound cardiography.The aforementioned three kinds of cases right ventricle Tei index and the contrasting comparative group average had marked difference.Conclusion Right ventricle Tei index is a relatively sensitive indicator for reflecting cardiac function as a whole.
