大剂量泼泥松联合沙利度胺治疗老年人多发性骨髓瘤的疗效观察

目的:探讨大剂量泼泥松联合沙利度胺治疗老年人多发性骨髓瘤的疗效。方法:将收治的80例老年性多发性骨髓瘤患者作为观察对象,随机分为观察组和对照组,各40例,观察组采用大剂量泼泥松联合沙利度胺治疗;对照组采用小剂量泼泥松联合沙利胺治疗;观察比较两组患者的临床疗效、血红蛋白、骨痛评分、生活质量评分和不良反应。结果:两组患者均顺利完成治疗,治疗组CR为0例,PR为31例,SD为6例,PD为3例,有效率为77.5%;对照组CR为0例,PR为20例,SD为11例,PD为9例,有效率为50.0%;治疗组有效率明显高于对照组,且差异具有统计学意义(P<0.05);治疗后两组的血红蛋白、骨痛评分和生活质量评分均较治疗前明显好转,且观察组改善情况明显优于对照组,差异具有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:大剂量泼泥松联合沙利度胺可以有效地治疗老年人多发性骨髓瘤,同时不增加不良反应,值得推广应用。

Clinical Effect Observation of Large Dose of Prednisone combined Thalidomide for Treatment of Senile Multiple Myeloma

Objective To discuss the clinical effect of large dose of prednisone combined thalidomide for treatment of senile multiple mye-loma. Method 80 patients with senile multiple myeloma were chosen as objects of observation and randomly divided into observation group and control group both with 40 cases. Observation group was given large dose of prednisone combined thalidomide for treatment while control was given small dose of prednisone combined thalidomide for treatment. Clinical effects,hemoglobin,bone pain scores,life equality scores and adverse reactions of the 2 groups were observed and compared. Results Both groups completed treatments successfully,there was no case of CR,31 cases of pR,6 cases of sD and 3 cases of pD in treatment group,effective rate was 77. 5%;there was no case of CR,20 cases of pR,＆amp;nbsp;11 cases of sD and 9 cases of pD in treatment group,effective rate was 50. 0%（P〈0. 05）;hemoglobin,bone pain scores,life equality scores of the 2 groups both improved compared with before treatments and observation group was obviously better（P〈0. 05）;no significant differ-ence existed between occurrence rates of complications of the 2 groups（P＆gt;0. 05）. Conclusion Large dose of prednisone combined thalido-mide can effectively cure senile multiple myeloma without increasing adverse reactions,so it is worth of being popularized in application.
