序贯性营养支持对急性有机磷农药中毒合并呼吸衰竭患者的疗效观察

目的:观察肠外营养(PN)、肠内营养(EN)序贯治疗对急性有机磷农药中毒合并呼吸衰竭患者的疗效观察。方法:将32例急性有机磷农药中毒合并呼吸衰竭患者随机分为两组,观察组给予TPN治疗,对照组给予早期PN+EN序贯治疗,观察两组患者入院时、治疗2周后的血清前白蛋白、血清白蛋白、血清总蛋白的变化,并比较两组患者治疗过程中的机械通气时间、并发症、病死率。结果:观察组患者治疗2周后前白蛋白、血清白蛋白、血清总蛋白较治疗前提高,差异有统计学意义(P<0.05),且明显高于对照组患者的同期营养指标,差异有统计学意义(P<0.05)。观察组患者的机械通气时间、消化道并发症、病死率明显低于对照组,差异有统计学意义(P<0.05)。结论:急性有机磷农药中毒合并呼吸衰竭的患者,早期运用PN+EN序贯治疗,提高了患者的营养状况,降低了患者的病死率。

Sequential nutritional support for acute organophosphorus pesticide poisoning effect observed in patients with respiratory failure

Objective Observation of parenteral nutrition（ PN）,enteral nutrition（ EN）in sequential therapy of acute organic phos-phorus pesticide poisoning effect observed in patients with respiratory failure. Method 32 cases of patients with acute organophosphorus pesticide poisoning complicated with respiratory failure were randomly divided into two groups,the observation group treated with TPN,con-trol group was given early PN﹢EN sequential treatment,changes were observed in two patients at admission,2 weeks after the treatment,se-rum albumin,serum albumin,serum total protein,and compare the two groups of patients in the process of mechanical ventilation time,com-plications,mortality rate. Results The patients in observation group after two weeks of treatment,pre albumin,serum albumin,serum total protein increased compared with before treatment,the difference was statistically significant（ P﹤0. 05 ）,and significantly higher than the same period the nutritional index in patients with the control,the difference was statistically significant（P﹤0. 05）. The observation group of patients with mechanical ventilation time,gastrointestinal complications,mortality rate was significantly lower than the control group,the difference was statistically significant（ P﹤0. 05）. Conclusion Combined with respiratory failure in acute organophosphorus pesticide poi-soning patients,the early use of PN﹢EN sequential treatment,improve the nutritional status of the patients,reduce the mortality of patients.
