摘    要:目的 探讨基于非酒精性肝硬化的轻微型肝性脑病(MHE)患者自发脑活动改变情况。方法 收集14例MHE患者(MHE组)、17例无肝性脑病的肝硬化患者(NHE组)和14名健康对照者(HC组),行静息态下fMRI扫描,分别获取3组受试者低频振幅(ALFF)并进行统计学分析。结果 与HC组相比,MHE组患者在右小脑半球后叶、左岛盖部额下回、右额前内侧回、右楔前叶、左枕中回ALFF降低,而右颞下回、左小脑半球前叶ALFF升高(P均<0.05);与NHE组相比,MHE患者右小脑半球后叶、左岛盖部额下回和右额前内侧回ALFF降低,仅右颞下回ALFF升高(P<0.05)。结论 基于非酒精性肝硬化的MHE患者存在相关脑区自发神经元活动异常。

Abstract:Objective To explore the abnormality of spontaneous brain activity in patients with minimal hepatic encephalopathy (MHE) based on non-alcoholic cirrhosis using fMRI. Methods Totally 14 patients with MHE (MHE group), 17 cirrhotic patients without hepatic encephalopathy (NHE group) and 14 healthy contrasts (HC group) underwent resting-state fMRI, and the amplitude of low-frequency fluctuation (ALFF) analysis was performed. Results Compared with that of HC group, ALFF decreased in the posterior lobe of right cerebellum, the opercular region of left inferior frontal gyrus, the right medial prefrontal cortex, the right precuneus and the left middle occipital lobe, but increased in the right inferior temporal lobe and the anterior lobe of left cerebellum in MHE group (all P<0.05). Meanwhile, compared with that of NHE group, ALFF decreased in the posterior lobe of right cerebellum, the opercular region of left inferior frontal gyrus and the right medial prefrontal cortex, but just increased in the right inferior temporal lobe in MHE group (P<0.05). Conclusion Abnormality of spontaneous brain activity exists in the related brain regions of patients with MHE based on non-alcoholic cirrhosis.
