外固定支架治疗胫骨干骨折骨不连

目的：探讨外固定支架在胫骨干骨折骨不连治疗中的应用方法，并对其疗效进行评估。方法：2000年7月至2009年3月应用外固定支架治疗胫骨干骨折骨不连62例，按Iowa标准对患者术前术后膝踝关节功能进行评价。结果：本组术后随访8个月～4．4年（平均2．6年），除6例因术后过早下地活动，导致外固定支架针道松动，需再次手术，其余病例均愈合，愈合率90．32％。术后膝踝关节功能恢复改善，按Iowa标准评价，患者术前术后膝踝关节功能经统计学处理，差异具有统计学意义（P〈0．05）。结论：外固定支架治疗胫骨干骨折骨不连，具有固定牢靠，有效维持肢体长度，早期活动，并发症少等优点。

Treatment of Nonunion of Tibia Shaft after Fracture with External Fixation Apparatu

Objective： To explore retrospectively the using techniques of external fixation apparatus in the treatment of nonunion of tibia shaft and evaluate the outcomes of the treatment. Methods： Sixty two cases suffered from nonunion of tibia shaft after fracture between July 2000 and March 2009 were treated by external fixation protocol. The function of the knee joint and ankle joint were judged basing on the Iowa criterion both pre - operatively and post - operatively. Results： After an average 2. 6 years of follow - up ranging from 8 months to 4. 4 years, 56/62 cases had achieved a osteal healing of the nonunion, the healing rate was 90. 32%. 6/62 cases had exchanged another interlocking intramedallary nail protocol due to walking burdened too early lead to the loosing of external fixation apparatus. The function of the knee joint and ankle joint had achieved a satisfactory recovery basing on the Iowa criterion. The differences of the function of the knee joint and ankle joint had a statistical significance post - operatively compared with pre - operatively. Conclusion： external fixation protocol have some advantages, such as providing firm fixation , feasible early activity, effective maintaining the limb length, less complications in the treatment of nonunion of tibia shaft after fracture.
