创伤性气管、支气管断裂的诊治

目的探讨创伤性气管、支气管断裂损伤的诊断与治疗。方法回顾性分析8例创伤性气管、支气管断裂患者诊治资料。结果本组5例行支气管端端吻合术;2例行支气管修补术,1例保守治疗。术后多次纤维支气管镜检查,其中1例因支气管针尖样狭窄行左下肺叶切除术,余均通畅良好,肺复张满意。结论创伤性气管、支气管断裂易误诊,胸部CT及纤维支气管镜检查是早期诊断的重要手段。一经确诊应立即手术,首选支气管重建术。术后宜早期复查纤维支气管镜。

Diagnosis and treatment of traumatic bronchial rupture

[Objective] To investigate the diagnosis and treatment of traumatic bronchial rupture. [Methods] A group of 8 cases of traumatic bronchial reconstruction was studied. [Results] End-to-end anastomosis was performed in 5 cases, and two received bronchial repair. Conservative treatment was performed on 1 case. One patient underwent left down pulmonary lobectomy because of stricture of anastomosis post-operation. The remaining obtained satisfactory results. [Conclusion] Early diagnosis and operation in such cases can be life-saving, and may warrant early recovery of the pulmonary function. Computer tomography and bronchoscopy play a key role in establishing the diagnosis. Bronchial reconstruction is optional for surgical treatment.
