角膜塑形镜矫正青少年近视的效果观察

目的探讨角膜塑形镜矫正不同类型(低度近视和中度近视;高曲率组和低曲率组近视)青少年近视的效果。方法选取2009年至2011年在我院就诊患者54人,共102只眼,年龄8～16岁,平均11.24±3.56岁,性别不限;等效球镜度数-1.00～-6.00D,顺规散光<-1.50D,逆规散光<-0.75D,配戴夜戴型角膜塑形镜,戴镜时间8～10h,并按照近视度数和曲率高低分为,低度近视组和中度近视组,低曲率组(≤43D)和高曲率组(43D∽46D);观察戴镜后第1d、1周、2周、1月、2月、3月、6月、12月后患者视力、屈光度、曲率变化情况并进行比较;对所有结果应用SPSS11.5统计软件进行分析。结果所有戴镜者白天视力都基本恢复正常(视力达1.0),其中低度近视组在戴镜后1周、中度近视组在戴镜后1月视力恢复最佳并稳定;所有患者角膜曲率都降低,并在戴镜后一月稳定,高曲率组比低曲率组曲率下降更大,低曲率组平坦曲率下降(2.06±0.78D),高曲率组平坦曲率下降(2.81±0.98D);低曲率组陡峭曲率下降(1.74±0.70D),高曲率组陡峭曲率下降(2.84±1.09D);结论角膜塑形镜是矫正青少年中、低度近视的有效方法,低度近视及高曲率者效果更佳。.

The effect of orthokeratalogy correcting the myopia in adoiescents

Objective To study the effect of orthokeratalogy on controling the development of myopia in adoIescents.Methods Fifty four adolescents（102 eyes in all）wjth myopia were included in this study.The average age was（11.24±3.56）years.The average spherical equivalent refraction was-1.0～-6.0D.Shun-rule astigmatism was less-1.5D and against the rule astigmatism was less than-0.75D,All patients were divided into two groups according to refraction and corneal curvature,low myopia group and moderate myopia group,low corneal curvature group（≤43D） and high corneal curvature group（43D,46D）.All patients were fitted with overnight orthokeratalogy for 8～10 hours at night.The corrected visual acuity,refraction and corneal curvature were measured at 1day,1 week,2weeks,1month,2months,3months,6months,12months afrer wearing orhokeraoIogy lenses.All results were analysed by SPSS11.5.Results The corrected visual acuity of all wearers reached 1.0.Wearers of low myopia group got and keeped the best corrected visual acuity at 1 week after wearing orthokeratalogy,but wearers of moderate myopia group at 1month.Corneal curvature of all wearers decreased and almost did not changed at 1 month after wearing orthokeratalogy.Corneal curvature of high corneal curvature group decreased more quickly compared with low corneal curvature group.The horizontaI curvature of low corneal curvature group decreased 2.06±0.78D to 2.81±0.98D of high corneal curvature group,the vertical curvature of low corneal curvature group decreased 1.74±0.70D to 2.84±1.09D high corneal curvature group.Conclusions Orthokeratalogy can correct low and moderate myopia in adoIescents effectively,low myopia and high corneal curvature were better.
