通脉灵对心肌缺血再灌注犬心功能的保护作用

摘    要:本实验复制24只杂种犬心肌缺血再灌注损伤病理模型,应用与生理盐水、丹参对比的方法观察通脉灵对血液动力学及心功能的影响。结果表明,通脉灵小剂量组(TM1G)、通脉灵大剂量组(TM2G)和丹参对照组(DSG)的心输出量(CO),心脏指数(CI),心搏指数(SI)、左室内压峰值(LVSP)及左室内压最大变化速率(±dp/dtmsx)均明显高于生理盐水对照组(NSG)。本实验证明,通脉灵主要是通过保护心肌舒缩功能改善血液动力学指标进而防止缺血再灌注时心功能不全的发生。

Abstract:To determine the protective effects of Tong一Mai一Ling on myocardial reperfusioninjury,24 hybrid dogs were divided randomly into 4 groups ,the open一chest dogs were subject to leftanterior coronary descending artery occlusion for 60 min followed by 4 h reperfusion. Dogs were ran-domized treated with Tong一Mai一Ling一l(TM,Tong一Mai一Ling一2(TM2),Dan一Shen(DS)and norn1al saline(NS)at 0.5h of occlusion,Cardiac function and hemodvnamic variables were measured before occlusion 60 min after occlusion,and l h,2h,4h after reperfusion. Cardiac output(CO),cardiac index(CI),left ventricle work index(LVWI ) ,heart stroke index(SI ),±dp/dtmaxand leftventricular pressure(LVSP)markedly decreased ,while total peripheral vascular resistance(TPVR )and left ventricular endodiastolic pressure(LVEDP)increased with the development of occlusion一reperfusion in NS group.TM1,TM2 and DS increased the mean blood pressure(MBP ),CO , CI ,SI,LVWI , LVSP and±dp/dtmax,while decreased TPVR,LVEDP after occlusion一reperfusion in com-parison with NS group. These results suggested that TML reduced myocardial ischemic reperfusion in-jury with salvage of myocardium,improved cardiac function and some indexes of hemodynamics signif-icantly ,and prevented heart from myocardial ischemia。
