PDCA循环法对急诊标本周转时间合格率的影响

目的 探讨运用PDCA循环法后，急诊检验标本周转时间（TAT）合格率的变化。 方法 统计分析西北妇女儿童医院医学检验中心2022年1月至4月共8 455例急诊标本报告，统计标本TAT合格率并采用鱼骨图分析不合格原因及处理措施；统计运用PDCA循环法后2022年5月至2023年1月共20 140例急诊标本报告，比较PDCA循环法实施前后急诊标本TAT合格率的变化。 结果 PDCA实施后，急诊标本TAT合格率高于PDCA实施前，差异有高度统计学意义（P＜0.001）。其中不同急诊项目中，除免疫发光外，急诊血常规、尿常规、粪常规、血凝四项和生化标本TAT合格率均较PDCA循环前升高，差异有高度统计学意义（P＜0.001）。 结论 通过PDCA循环管理法，西北妇女儿童医院医学检验中心急诊标本TAT合格率得到了明显提升，这不仅提升了检验效率和临床服务，同时对急、危重患者的及时救治具有重要意义。

Effect of the PDCA cycle method on report trun-around-time pass rates in emergency medicine testing

Objective To investigate the changes in report turn-around-trim (TAT) pass rates for emergency specimens after applying the PDCA cycle. Methods Statistical analysis of a total of 8 455 emergency samples reported to the Medical Laboratory Centre, Northwest Women’s and Children’s Hospital from January to April 2022; the report TAT pass rates were counted and the fishbone diagram was used to analyze the reasons for failure and treatment interventions; a total of 20 140 emergency specimens reports were counted from May 2022 to January 2023 after the application of the PDCA cycle method, and the changes in report TAT pass rates before and after the implementation of the PDCA cycle method were compared. Results After the implementation of PDCA, the pass rate of emergency specimen TAT was higher than that before the implementation of PDCA, and the difference was highly statistically significant (P＜0.001). Among different emergency procedures, except for immunoluminescence, the pass rates of emergency blood routine, urine routine, fecal routine, blood coagulation, and biochemical specimen TAT were all higher than those before the PDCA cycle, and the differences were highly statistically significant (P＜0.001). Conclusion Through the PDCA cycle method, the pass rate of the emergency sample TAT of the Medical Laboratory Centre, Northwest Women’s and Children’s Hospital has been significantly improved, which not only improves the testing efficiency and clinical services, but also plays an important role in the timely treatment of emergency and critical patients.
