宁波市不同时期肺结核病耐药监测结果分析

目的分析宁波市不同时期肺结核患者耐药状况,为结核病控制规划的顺利实施提供科学依据。方法回顾分析宁波市1999年10月-2000年3月开展的耐药监测资料（设为Ⅰ组）及2009年7月-2009年12月开展的耐药监测资料（设为Ⅱ组）。对组间耐药情况和不同抗结核药物的耐药顺位进行比较分析。统计学处理采用SPSS 13.0统计软件,率的比较用χ^2验,P〈0.05为差异有统计学意义。结果Ⅰ组肺结核病例总耐药率为22.4%（57/254）,Ⅱ组肺结核病例总耐药率为32.1%（124/386）,总体耐药率有上升趋势,经统计学处理,差异有统计学意义（χ^27.082,P〈0.05）;Ⅰ组肺结核病例耐多药率为6.3%（16/254）,Ⅱ组肺结核病例耐多药率为8.0%（31/386）,两组比较,差异无统计学意义。结论宁波市肺结核病耐药形式依然严峻,今后须进一步加强耐药结核病的防治工作。

Drug-resistance in patients of pulmonary tuberculosis in different periods

Objective To provide reasonable evidence of chemotherapy for patients with pulmonary tuberculosis based on the prevalence of drug resistance in different periods. Methods Two groups of patients from different periods of time were selected for the study. Group Ⅰ were patients with smear positive tuberculosis from October 1999 to March 2000, and Group Ⅱ were patients with smear positive tuberculosis from July 2009 to December 2009. Drug resistance data was analyzed retrospectively between the two groups. Results The rates of drug resistance were 22.4% （57/254） in group Ⅰ and 32. 1% （124/386） in group Ⅱ. There was significant statistical difference on the rate of drug resistance （Х^2= 7. 082, P〈0. 05）. The rates of multi-drug resistance were 6.3% （16/254） in group Ⅰ and 8.0% （31/ 386） in group Ⅱ. Conclusion The results showed that the prevalence of drug-resistant TB was severe in Ningbo, Zhejiang province, and the tuberculosis control program should be strengthened.
