深圳市公明医院高血压、糖尿病病人双向转诊调查分析

目的开展高血压、糖尿病病人双向转诊的实证研究，获取有关证据，为卫生管理和医院管理提供决策证据。结果“两病”病人5年平均双向转诊率为6．77％，从2003年的0．2％逐年上升到2006年的15．86％；转诊分布具有季节特征，7-9月份为高峰；转诊病人年龄集中在30～、40～、50～3个年龄段。“两病”下转病人分别流向21个社康服务站（中心），平均为16．57人／站。距离公明医院近的社康服务机构的转诊病人显著高于距离公明医院远的社康服务机构。结论深圳公明医院与其下属的20多家社康服务机构之间两病管理制度已经较好地开展了双向转诊工作，取得了较好的成效。

Survey of the Dual Referral Patients with Hypertension and Diabetes in Gongming Hospital of Shenzhen

Objective To access to the related evidence for health management and hospital management's decision-making by empirical survey of the dual referral patients with hypertension and diabetes.Results The average referral proportion of hypertension and diabetes patients was 6.77% during the last five years,and the referral proportion rose from 0.2% in 2003 to 15.86% in 2006.The distribution of referral patients has Seasonal features,with the peak appeared from July to September.Most of the patients were over 30-year-old,40-year-old and 50-year-old.21 institutions of community health services(CHSs) received the referred patients who were transformed down from the hospital,and each CHS received 16.57 patients averagely.The referral patients in the CHSs which is mear to Gongming Hospital were much more than those in the CHSs which is father from it.Conclusion Gongming Hospital and its ruled CHSs perform the better dual referral mechanism and achieved good effect.
