自拟利胆方辅助治疗妊娠期肝内胆汁淤积症的临床疗效研究

目的 观察自拟利胆方对妊娠期肝内胆汁淤积症（ICP）的辅助治疗作用.方法 将2009年1月-2012年10月在该院住院治疗的ICP患者70例随机分为观察组和对照组.两组均给予ICP的常规治疗及监护,观察组在此基础上每天加服自拟利胆方汤剂2次.分别于治疗3 d、7 d后,观察比较两组患者血清中相应生化指标的水平,皮肤瘙痒,妊娠结局及围产儿情况.结果 治疗3 d结束后,两组患者相应生化指标及皮肤瘙痒评分与治疗前相比,均差异有显著性（P＜0.05或P＜0.01）;治疗7 d结束后,两组患者血清中相应指标及皮肤瘙痒评分改善程度更加明显,且观察组优于对照组（P＜0.05或P＜0.01）;治疗7 d结束后,观察组总有效率达94.11%,对照组总有效率达77.78%,且两组相比差异有显著性（P＜0.05）;两组患者妊娠结局及围产儿情况相比,除分娩孕周及剖宫产率有显著性差异外（P＜0.05）,其余指标则差异无显著性（P＞0.05）.结论 自拟利胆方辅助治疗妊娠期肝内胆汁淤积症有较好的临床疗效,值得推广.

Assistant efficacy of self-made lidanfang in the treatment of Intrahepatic cholestasis of pregnancy

Objective To investigate the assistant efficacy of Self-made Lidanfang in the treatment of Intrahepatic Cholestasis of Pregnan- cy（ICP）. Methods 70 patients with ICP in our hospital treatment from January 2009 to October 2012 were randomly divided into obser- vation group and control group. The participants in both groups were given the conventional treatment and care of ICP, and the partici- pants in observation group took Self-made Lidanfang in addition twice a day. Compare the levels of blood biochemical criterion, cutaneous pruritus, pregnancy outcome and perinatal infant between the two groups after treatment 3 days and 7 days respectively. Results After 3 days treatment, the levels of blood biochemical criterion and cutaneous pruritus in both groups were different signifcantly（ P 〈0.05 or P 〈 0.01 ） ; After 7 days treatment, the blood biochemical criterion and cutaneous pruritus were improve more significantly, and the obser- vation group is better than control group（P 〈0.05 or P 〈0.01 ） ; After 7 days treatment, the total effective rates of observation group is 94.11%, the total effective rates of control group is 77.78 %, and the two groups had signifcant difference （ P 〈 0.05 ） ; Except the preg- nant weeks and the cesarean section rates of the two groups had signifcant difference（ P 〈 0.05 ）, the rest indices of pregnancy outcome and perinatal infant in both groups had no signifcant difference （ P 〉 0.05 ）. Conclusions Self-made Lidanfang had good assistant effi- cacy in the treatment of Intrahepatic Cholestasis of Pregnancy, and it is worthy to spread.
