心肌组织多普勒评价急性下壁心肌梗死的右心功能

目的　应用心肌组织多谱勒技术 (TDI)结合 M型超声评价急性下壁心肌梗死 (AIWMI)患者右心室整体功能。方法　正常对照组 2 0例 ,AIWMI2 0例 ,在标准心尖四腔心切面 ,采用 M超记录三尖瓣环右心室游离壁处运动曲线 ,测量右心室收缩期、舒张早期与晚期最大运动幅度 (SD、DED、DAD)及其平均速度 (SV、DEV、DAV ) ,求出舒张早期和舒张晚期最大运动幅度比值 (DED/ DAD)。应用 TDI技术测量上述各期峰值运动速度 (Sm、Em和 Am ) ,求出舒张早期峰值运动速度 (Em)和舒张晚期峰值运动速度 (Am )的比值 (Em / Am )。结果　与对照组相比 ,AIWMI组 SD、DED、SV、DEV、DED/ DAD均显著降低 ,Sm、Em及 Em/ Am比值也显著降低。结论　 TDI可以评价心肌梗死后右心室整体功能

Right ventricular function in patients with acute inferior wall myocardial infarction:assessment by tissue Doppler imaging

Objective To evaluate the whole RV function with tissue Doppler imaging and M-mode echocardiography in tricuspid annulus motion. Method Twenty patients with a first acute inferior MI were prospectively compared with 20 age-matched healthy individuals. With the use of 2-dimentional guided M-mode echocardiography, the motion of the tricuspid annulus at the RV free wall was recorded in apical 4-chamber views, then the altitudes and mean velocities of systole(SD,SV) and early and late diastole (DED,DEV,DAD,DAV) was measured and the ratio of altitudes of early and late diastolic period(DED/DAD) was calculated .Peak systolic and early and late diastolic velocities (Sm,Em,Am) of the tricuspid annulus was also recorded at the same site by tissue doppler imaging and the ratio of peak early and late diastolic velocities(Sm/Am) was counted too. Results Compared with that in healthy individuals,the SD,DED,SV,DEV,DED/DAD was reduced dramasticaly in inferior MI, so the Sm,Em,Sm/Em was in inferior MI. Conclusion Tissue Doppler imaging can be used to assess the whole impared RV function in association with the inferior MI.
