液基薄层细胞学检查和巴氏涂片在诊断宫颈恶性病变中的比较研究

目的比较液基薄层细胞学技术（TCT）和巴氏涂片在诊断宫颈恶性病变中的应用价值。方法收集132例拟诊为富颈恶性病变的患者，对其进行TCT和巴氏涂片检查，并与手术病理结果进行对照。结果TCT检查和巴氏涂片标本同时合格109例，合格率82．58％。经手术或活检病理学诊断证实，其中宫颈癌86例、宫颈炎性改变7例、CINI4例、CIN II 6例、CINm4例、其他2例。TCT检查灵敏度为93．02％，特异度为65．22％，阳性预测值为90．91％，阴性预测值为71．43％，符合率为87．16％；巴氏涂片灵敏度为86．05％，特异度为56．52％，阳性预测值为88．10％，阴性预测值为52．00％，符合率为79．82％。TCT检查灵敏度、特异度、阴性预测值、符合率均优于巴氏涂片。结论TCT检查和巴氏涂片整体质量均加好，但是TCT检查灵敏度、特异度、阴性预测值、符合率优于巴氏涂片。

The Comparison Rearch of Thip-Prep Cytology Test and Pap Smear in the Diagnosis of Cervical Malignant Disease

Objective To compare the application value of Thip-Prep Cytology Test （TCT） and Pap Smear in the diagnosis of cervical malignant disease. Methods We collected 132 cases of patients with suspicious cervical malignant disease. TCT and Pap Smear were conducted to them which were compared with pathological results. Results 109 cases of Specimens from TCT and Pap Smear were qualified simultaneously, the qualified rate was 82.58%. Confirm by operative and biopsy pathological results, there were 86 cases of cervical cancer, 7 cases of inflammatory lesions, 4 cases of CIN I, 6 cases of CIN 11,4 cases of CINIII and 2 cases of other disease. TCT： Sensitivity （Se） was 93.02%, specificity （Sp） was 65.22%, positive predictive value （＋PV） was 90.91%, positive predictive value （-PV） was 71.43%, agreement rate was 87.16%. Pap Smear： Se was 86.05%, Sp was 56.52%, ＋PV was 88.10%, -PV was 52.00%, agreement rate was 79.82%. The Se, Sp, PV and agreement rate of TCT were better. Conclusion The quality of TCT and Pap Smear were excellent, but The Se, Sp, PV and agreement rate of TCT were better than Pap Smear.
