下颈段骨折脱位病人手术方法选择及其适应证

摘    要:目的 探讨下颈段骨折脱位病人手术方法选择及其适应证。方法 对23例下颈段骨折脱位病人根据其不同的损伤机制和术前X线,MR检查特点,分别采用前路AO钛板固定或后路张力钛缆固定或前路AO钛板+后路张力钛缆固定方法手术治疗。结果 全组病人获短期随访,平均8.1个月,病人均能早期活动,进行康复锻炼,无任何早期并发症。

Abstract:Objective To Study the surgical techniques and indication in treatment oflower cervical spine fractures and fracture-dislocation. Methods According to the char- acteristics of cervixal injury mechanism and preoperative X-ray and MRI examination, 23patients with lower celtical spine fractures and fracture-dislocation were given surgicalinterventions of anterior AO titanium plate, posterior titanium cable tension band fixationand combination of the two methods. Results All patients were reviewed at an averagefollow-up of 8.1 months. They all got early mobilization and rehabilitation without anyearly complications. During the follow-up after operation, the Frankel Scale changed asthe followings: 9 cases of scale A (9A) chaded to 4A, 3B and 2C; 7B changed to 1B, 4Cand 2D; 6C chanced to IC, 4D and 1E; 1D changed to 1E. Conclusions Anterior AOtitanium plate and POSterior titanium cable tension band fixation is the optimal surgicalmethods in treatment of cervical spine fracture and fracture-dislocation. Anterior AOtitanium plate fixation is adapt to those who have anterior column lesions in cervicalspine or anterior spinal cord pressed. Posterior titanium cable tension band fixation isadapt to those who have posterior column lesions.The combination of these two methodsis adapt to those who have both anterior and posterior column lesions in cervical spine.
