脑梗死患者颈动脉斑块与血浆同型半胱氨酸水平及亚甲基四氢叶酸还原酶基因多态性的关系

摘    要:目的探讨血浆同型半胱氨酸水平、亚甲基四氢叶酸还原酶(MTHFR)基因多态性与脑梗死患者颈动脉斑块之间的关系。方法选择颈动脉系统急性脑梗死患者128例,采用荧光偏振免疫法测定血浆同型半胱氨酸水平,聚合酶链反应-限制性片长多态性技术检测MTHFR基因多态性。应用彩色超声心动仪进行颈动脉超声检查,根据超声结果将患者分为颈动脉粥样硬化斑块组(简称斑块组)和无斑块组,比较两组血浆同型半胱氨酸水平及MTHFR基因多态性。结果斑块组TT基因型(35.8%比17.0%)及T等位基因频率(58.6%比40.4%)均显著高于无斑块组(P<0.05)。斑块组血浆同型半胱氨酸水平显著高于无斑块组(22.42±11.04μmol/L比17.89±5.96μmol/L,P<0.05)。各组内MTHFR 677TT纯合子同型半胱氨酸水平均显著高于CT型和CC型者(P<0.05)。Logistic回归分析表明,校正性别、年龄、高血压、糖尿病、高脂血症、吸烟、饮酒等危险因素后,血浆同型半胱氨酸水平仍为颈动脉粥样硬化斑块的独立危险因素[OR 1.160(95%CI 1.034~1.301),P<0.05]。结论血浆同型半胱氨酸水平升高是脑梗死患...
