胃癌患者外周血淋巴细胞免疫功能的研究

目的检测胃癌患者外周血淋巴细胞免疫功能状态,探讨其与胃癌新临床病理分期(TNM)及组织病理学类型的关系.方法流式细胞仪测定胃癌患者外周血T淋巴细胞亚群及NK细胞数量,以胃溃疡患者及正常人作对照分析.结果胃癌患者外周血CD3 、CD4 、NK细胞数量及CD4 /CD8 比值较正常对照组和胃溃疡组均明显下降(P＜0.01),而CD8 细胞水平显著升高.T淋巴细胞亚群及NK细胞数量的改变与胃癌组织病理学类型无关,而与胃癌临床病理分期有关,分期越晚,CD3 细胞、CD4 细胞、CD4 /CD8 细胞比值及NK细胞数量越低,CD8 细胞水平越高;在Ⅰ、Ⅱ期胃癌患者与Ⅲ、Ⅳ期患者之间存在显著差异(P＜0.05或P＜0.01).结论胃癌患者外周血淋巴细胞免疫功能低下,且临床病理分期越晚,其免疫功能越低.积极的外科手术治疗以及免疫治疗有助于恢复其免疫功能.

Study on lymphocyte immune function in peripheral blood of the patients with gastric cancer

Objective To analyze the relationship of peripheral blood lymphocytes immune function of the patients with gastric cancer to the new clinical pathological stage(TNM) and the histological types of the tumors.Methods Flow cytometry was used to detect the marks of T lymphocytes subsets(CD3 ,CD3 CD4 ,CD3 CD8 ) and the NK cells(CD3 -CD16 56 ) cells in blood samples from patients with gastric cancer.Data obtained from health people and patients with gastric ulcer was used as control.Results Lower CD3 ,CD4 T cells,NK cells and CD4 /CD8 ratio were detected in blood samples from cancer group than that from ulcer group and health control( P <0.01).CD8 T lymphocytes were reverse.This change is related to the TNM pathological stage,not to the histological types of the tumors.Conclusions The immune function of lymphocytes in peripheral blood of the patients with gastric cancer was low,and even worse in the lower grade of TNM stage.Positive operation and immunotherapy post operation may be valuable for these patients.
