摘    要:目的检测干细胞因子(Stem cell factor,SCF)在人自体移植中厚皮片中的表达,初步认识干细胞因子在自体移植皮片色素沉着发生机制中的作用。方法取面颈部自体中厚皮片移植术后1年左右的色素沉着皮片,以自身正常皮肤作为对照,组成配对资料(n=16),利用S—P免疫组化方法,检测SCF在受区移植皮片、受区周围与供区周围正常皮肤的表达与分布,使用MIQAS医学图像定量分析系统,定量检测各组标本中SCF阳性表达面积(PA)、阳性表达强度(OD),计算出阳性表达指数(PI,PI=PA×OD),并就PI值行统计学分析。结果SCF表达定位于表皮基底层的黑素细胞与角朊细胞的胞浆及胞膜表面。SCF在受区移植皮片、受区周围及原供区周围正常皮肤中的PI值分别为158830±53753、86839±21381、85033±16972。SCF在受区移植皮片组的表达与两正常对照组相比具有显著性差异(p〈0.01);SCF在受区周围正常对照皮肤与原供区周围正常皮肤中的表达无差异。结论SCF在自体中厚移植皮片中的表达较在自体正常对照皮肤中显著增高,推测SCF在自体移植皮片色素过度沉着中可能起重要调控作用。

Expresslion of Stem Cell Factor in Human Intermediate-split-thickness Autograft Skin

Abstract:Objective To explore the role of stem cell factor (SCF) in hyperpigmentation process of the human split-thickness grafted skin by assaying the expression of SCF. Methods With self normal skin as control, we get pairing data (n= 16) by cutting facial and cervical intermediate split thickness skin grafts which showed pigmentation post operation about 1 year. SCF expression and distribution was detected by S-P immunohistochemical reaction of following three groups: the auto transplanted skin grafts group, the normal skin around the recipient site group, and the normal skin around the donor site group. Quantitative analysis of SCF expression in these three groups was measured with Medical Image Quantitative Analysis System (MIQAS) by calculating the Positive Expression Area (PA) and Positive Expression Intensity (OD). Positive Expression Index (PI), which was the product of PA and OD, was used to have statistics analysis. Results SCF was mainly expressed in endochylema and membranes of melanocytes and keratinocytes of palisade layer. PI of above three group respectively were 158 830 ± 53753,86 839 ± 21 381, and 85 033 ± 16 972. SCF expression in autograft split-thickness skin was significantly higher than that of normal skin (p〈0.01). There was no difference of SCF expression between the normal skin around the recipient site and the normal skin around the donor site. Conclusion SCF expression in split-thickness grafted skin increased greatly compared with that in control groups of normal skin. SCF may play an important role in regulating hyperpigmentation of the autograft skin.
